3 English English INTENDED USE The MESACUP anti-skin profile TEST is a qualitative enzyme-linked immunosorbent assay (ELISA) for the detection of IgG antibodies to Dsg1, Dsg3, BP180, BP230 and TypeVII collagen in human serum. Five individual autoantibodies can be detected simultaneously in a single assay. The MESACUP anti-skin profile TEST is intended for in vitro diagnostic use, as an aid in the diagnosis of certain autoimmune bullous diseases. SUMMARY AND EXPLANATION Autoimmune bullous diseases are a group of cutaneous disorders characterised by skin blistering or erosions and classified into pemphigus and pemphigoid. Pemphigus is an autoimmune bullous disease targeting skin and mucous membranes. It is divided into two major subtypes: pemphigus vulgaris (PV) and pemphigus foliaceus (PF). In patients with PV, anti-desmoglein 3 (Dsg3) antibodies are produced. Among PV patients, patients with mucosal-dominant PV have only anti-dsg3, and patients with mucocutaneous PV have both Dsg1 and Dsg3 antibodies, whereas anti-dsg1 positive but anti-dsg3 negative patients are diagnosed as PF (1). These anti-dsg antibodies consist of pathogenic and non-pathogenic antibodies. A recent study has shown that the pathogenic anti-dsg antibodies bind to mature Dsg antigens that are unmasked by proteolytic processing of immature Dsg (2). Bullous pemphigoid (BP) is an autoimmune subepidermal bullous disease characterized by the presence of IgG autoantibodies to the epidermal basement membrane zone in sera. Autoantibodies from patients with BP target two distinct antigens, a 180 kda transmembrane protein termed BP180 and a 230 kda intracellular protein termed BP230. Epitope studies for anti-bp180 antibodies have shown that non-collagenous 16a (NC16a) domain of BP180 is the major epitope recognized by the majority of BP sera (3-4). For anti-bp230 antibodies, its C-terminal domain (BP230-C) mainly reacts to BP patient serum, but some BP patient sera recognize only N-terminal domain of BP230 (BP230-N) (5-6). Epidermolysis bullosa aquisita (EBA) is an autoimmune bullous skin disease characterized by the presence of Type VII Collagen IgG autoantibodies. Type VII Collagen is composed of three identical α-chains. Each α-chain consists of a central collagenous (COL) domain flanked by a 145 kda non-collagenous amino-terminal (NC1) domain and a smaller 34 kda carboxyl-terminal non-collagen (NC2) domain. The major antigenic epitopes for anti-type VII collagen autoantibodies in patients with EBA reside within the NC1 domain. However, the recent research paper showed that NC2 domain is a minor antigenic epitope for these autoantibodies (7). From the above background, the recombinant antigens used by this profile kit were listed to the following table. 4 English PRINCIPLE The MESACUP anti-skin profile TEST is an ELISA that detects five anti-skin-specific antibodies (anti-dsg1, anti-dsg3, anti-bp180, anti-bp230 and anti-typevii collagen) in patient serum simultaneously. Microwells (8 wells) are separately coated with 5 different recombinant antigens. Specific antibodies in diluted serum will bind to the antigen-coated well (Sample incubation). The wells are then washed to remove unbound antibodies and other components. A conjugate of horseradish peroxidase-labeled anti-human IgG antibody is added to wells and incubated (Conjuate incubation). After another washing step, the peroxidase substrate is added and incubated for an additional period of time (Substrate incubation). Acid solution is then added to each well to terminate the enzyme reaction and to stabilize the color development. The test result is determined photometrically by measuring the absorbance and plotting the results. BRIEF ASSAY PROCEDURE <Sample incubation> Add 100 µl of diluted sample (1:101) to designated wells of the microwell (20-30 C) 30 min. plate according to schema Wash <Conjugate incubation> Add 100 µl of Conjugate reagent to each well (20-30 C) 30 min. Wash <Substrate incubation> Add 100 µl of Substrate to each well (20-30 C) 15 min Add 100 µl of Stop solution to each well Read absorbance Interprete the result REAGENTS AND STORAGE 1) Antigen Microwell strips 96 wells Microwell strips (12 x 8 wells) coated with 5 individual recombinant antigens. The strips are packed in a strip holder and sealed in a foil envelope with desiccant. 2) Calibrator 1 (0 U/mL) One vial containing 1.5 ml of assay diluent containing 0.09% sodium azide. Ready-to-use, make no further dilution. 3) Calibrator 2 (100 U/mL) One vial containing 1.5 ml of autoantibodies positive human serum in assay diluent containing 0.09% sodium azide. Ready-to-use, make no further dilution. 2

5 English 4) Conjugate reagent One vial containing 15 ml of horseradish peroxidase conjugated goat anti-human IgG antibody. Ready-to-use, make no further dilution. 5) Assay diluent One vial containing 50 ml of Tris buffer containing 0.09% sodium azide. Ready-to-use, make no further dilution. 6) Wash concentrate (20X) 7) Substrate One vial containing 50 ml of PBS with Tween 20 concentrate (20X). One vial containing 20 ml of 3,3',5,5'-tetramethylbenzidine dihydrochloride /hydrogen peroxide (TMB/ H 2 O 2 ). Ready-to-use, make no further dilution. 8) Stop solution One vial containing 20 ml of 0.25 mol/l sulfuric acid. Ready-to-use, make no further dilution. Both unopened and first opened kit components are stable until the labeled expiration date at 2-8 C. PRECAUTIONS (1) This product is for in vitro diagnostic use only. (2) Do not use the kit components beyond the stated expiration dates. (3) Avoid contact of the reagents with eyes, skin and clothing. Reagents on skin must be washed away with plenty of water. TMB is irritating and the Stop solution consists of 0.25 mol/l sulfuric acid, which is a poison and is corrosive. (4) Calibrator 2 is derived from human serum, in which HBs antigen, HCV antibody, HIV-1 and HIV-2 antibodies have not been detected. No test method, however, can guarantee the absence of these or any other infectious agents. These reagents and all patient samples should be handled as if they are capable of transmitting AIDS, hepatitis or any other infectious diseases. (5) Calibrator 1, Calibrator 2 and Assay diluent contain sodium azide (0.09%) as a preservative and must be handled with caution. Do not ingest or allow contact with skin or mucous membranes. Sodium azide may react with copper or lead in plumbing system to form explosive metal azides. Therefore, always flush with plenty of water when disposing materials containing sodium azide into a drain. (6) Calibrator 1, Calibrator 2, Conjugate reagent and Assay diluent contain materials of animal origin, taken from non-infected animals. These components, however, should be treated as potential biohazards in use and for disposal. (7) Do not mix reagents from other kits. (8) All reagents must be brought to room temperature (20-30 C) before starting the assay. (9) Do not expose the kit to direct sun during the test and storage. (10) Avoid microbial and cross contamination of reagents or samples. (11) Incubation temperatures above or below normal room temperature (20-30 C), shorter or longer incubation times and inaccurate dilutions may give erroneous results. (12) The wells must be properly rinsed with Wash solution to avoid false positive results. (13) Carefully pipette each sample and reagent to avoid cross contamination between microwells, avoid foaming. 3

6 English (14) All unused Microwell strips should be returned to the ziplock pouch, which must be carefully resealed to avoid moisture absorption. (15) Wash concentrate may become turbid at 2-8 C, this does not cause inconsistent results. (16) Materials used for the test should be disposed or treated as shown below. Soak in 2% final conc. glutaraldehyde solution for more than 1 hour, soak in 0.1% sodium hypochlorite solution (available chlorine: approx. 1,000 ppm) for more than 1 hour or autoclave at 121 C for more than 20 minutes. (17) The antibody values obtained from this assay are aids in the diagnosis only. Each physician must interpret these results in light of the patient s history, physical findings, and other diagnostic procedure. (18) In the event of damage to the protective packaging, the kit components should be treated or disposed of in accordance with the relevant instructions and regulations of each country. MATERIALS REQUIRED BUT NOT PROVIDED Microplate reader (wavelength: 450 nm, 620 nm/reference) Multichannel micropipette (e.g. 100 µl µl) Single channel pipette (e.g. 10 µl & 100 µl) Autowasher or washing bottle Deionized or distilled water One liter graduated cylinder for preparation of Wash solution Test tubes for patient sample dilutions (e.g. 1,000 µl) Disposable pipette tips Paper towels Microplate cover PROCEDURE PREPARATION OF REAGENTS 1. Bring all assay materials to room temperature (20-30 C) prior to use. 2. Microwell strips: One strip must be used per one patient serum. Remove required Microwell strips from the pouch and place them in the frame. Promptly return unused strips to the refrigerated storage. 3. Wash solution: Prepare 1:20 dilution of the Wash concentrate prior to use (ex. add 50 ml of Wash concentrate to 950 ml of distilled water). The diluted Wash solution is stable for 2 weeks at 2-8 C. 4. Do not dilute Calibrator 1, Calibrator 2, Conjugate reagent, Assay diluent, Substrate and Stop solution. These reagents are ready-to-use. PREPARATION OF SAMPLES 1. Use fresh patient sera. Samples should be tested as soon as possible after collection. If storage is needed, samples should be stored at -20 C or lower. Do not repeat freezing and thawing. 2. Dilute each patient serum 1:101 by adding 10 µl of serum to 1 ml of Assay diluent. * Diluted samples can not be stored. Dilute each patient serum for each assay. ASSAY PROCEDURE * Assay diluent may form precipitates, which does not cause inconsistent results. STEP 1. (SAMPLE INCUBATION) With a multichannel micropipette, transfer each 100 µl of Calibrator 1, Calibrator 2, diluted patient serum 4

7 English and Assay diluent into each designated well of the microwell plate according to the schema below. (Do not dilute Calibrator 1, Calibrator 2 and Assay diluent.) Pipetting should be completed as quickly as possible. Cover the wells with a microplate cover and incubate for 30 minutes at room temperature (20-30 C). STEP 2. (WASHING) Aspirate or discard the well contents. Fill the well with Wash solution and then completely aspirate or discard the contents. Wash 4 times. Tap the plate on a paper towel to remove any remaining Wash solution. When autowasher is used, wash 4 times. * Each laboratory is recommended to confirm its own appropriate washing times and other conditions. * Wash solution should be used at C. STEP 3. (CONJUGATE INCUBATION) Add 100 µl of the Conjugate reagent to each well with a multichannel micropipette. Cover the wells with the microplate cover and incubate for 30 minutes at room temperature (20-30 C). * Do not return the Conjugate reagent once taken out of vial. STEP 4. (WASHING) Wash the microplate as described in STEP 2. STEP 5. (SUBSTRATE INCUBATION) Add 100 µl of the Substrate to each well with a multichannel micropipette. * Do not return the Substrate once taken out of vial. Cover the wells with the microplate cover and incubate for 15 minutes at room temperature (20-30 C). STEP 6. (STOP REACTION) Add 100 µl of the Stop solution to each well with a multichannel micropipette. READING Read the absorbance of each well at 450 nm. If a dual wavelength plate reader is used, set the test wavelength at 450 nm and the reference at 620 nm. * Reading should be done within 20 minutes after stopping the reaction. * Before reading the plate, ensure that the bottom of the plate is clean and dry, and that no air bubbles are present on the surface of the liquid in the wells. 5

The assay is calibrated in relative arbitrary units. QUALITY CONTROL Each assay result should meet the following criteria. A 450 of the Assay control (well H): > 0.5 A 450 of the Calibrator 1 (well A): < 0.1 A 450 of the Calibrator 2 (well B): > 0.5 x 100 If any of these criteria are not met, the results are invalid and the test should be repeated. Before repeating the test, check the following points. Each laboratory is recommended to establish its own criteria for test interpretation based on the sample populations that are typically found. LIMITATIONS As with other diagnostic test procedures, the results obtained with the MESACUP anti-skin profile TEST serve only as an aid in the diagnosis and should not be interpreted as diagnostics in themselves. 11 Français Français BUT DU DOSAGE Le MESACUP anti-skin profile TEST est un dosage qualitatif d'immuno-adsorption par enzyme liée (ELISA: enzyme-linked immunosorbent assay) pour la détection dans le sérum humain des IgG dirigées contre les Dsg1, Dsg3, BP180, BP230 et le collagène de type VII. Cinq auto-anticorps différents peuvent être détectés simultanément dans un même essai. Le MESACUP anti-skin profile TEST est destiné au diagnostic in vitro et est une aide au diagnostic de certaines maladies auto-immunes bulleuses. RÉSUMÉ ET EXPLICATIONS Les maladies auto-immunes bulleuses sont un groupe de maladies cutanées caractérisées par la formation de bulles ou d'érosions au niveau de la peau. Elles sont classifiées en pemphigus et pemphigoïde. Le pemphigus est une maladie bulleuse auto-immune affectant la peau et les muqueuses. Il se divise en deux sous-types majeurs: le pemphigus vulgaire (PV) et le pemphigus foliacé (PF). Les patients atteints de PV produisent des anticorps anti-desmogléine 3 (Dsg3). Parmi les patients PV, ceux qui sont atteints d'un PV à composante muqueuse dominante n'ont que l'anti-dsg3 et ceux qui sont atteints d'un PV muco-cutané ont à la fois des anticorps anti-dsg1 et anti-dsg3. Les patients anti-dsg1 positifs et anti-dsg3 négatifs sont diagnostiqués comme atteints de PF (1). Ces anticorps anti-dsg sont des anticorps pathogènes et non pathogènes. Une étude récente a montré que les anticorps anti-dsg pathogènes se lient aux antigènes Dsg matures qui sont démasqués par un processus protéolytique des Dsg immatures (2). La pemphigoïde bulleuse (PB) est une maladie bulleuse subépidermique auto-immune caractérisée par la présence dans le sérum d'auto-anticorps IgG dirigés contre la zone de la membrane basale épidermique. Les auto-anticorps de patients atteints de PB ciblent deux antigènes: une protéine transmembranaire de 180 kda appelée BP180 et une protéine intracellulaire de 230 kda appelée BP230. Des études des épitopes ont montré que le domaine non collagéneux 16a (NC16a) de la BP180 est l'épitope principal reconnu par la majorité des anticorps PB présents dans le sérum (3-4). Le domaine C-terminal (BP230-C) des anticorps anti-bp230 réagit principalement avec le sérum de patient PB, mais certains sérums de patient PB ne reconnaissent que le domaine N-terminal de la BP230 (BP230-N) (5-6). L'épidermolyse bulleuse acquise (EBA) est une maladie bulleuse auto-immune de la peau caractérisée par la présence d'auto-anticorps IgG anti-collagène de type VII. Le collagène de type VII est composé de trois chaines α identiques. Chacune des chaines α consiste en un domaine central collagéneux (COL) flanqué d'un domaine amino-terminal non collagéneux de 145 kda (NC1) et d'un domaine non collagéneux carboxy-terminal plus petit de 34 kda (NC2). Les principaux épitopes antigéniques pour les auto-anticorps anti-collagène de type VII chez les patients souffrant d'eba se trouvent dans le domaine NC1. Une publication de recherche récente a cependant montré que le domaine NC2 est un épitope antigénique mineur pour ces auto-anticorps (7). S'appuyant sur le contexte décrit ci-dessus, les antigènes recombinants utilisés par cette trousse profile sont repris dans le tableau suivant. 12 Français PRINCIPE DE LA MÉTHODE Le MESACUP anti-skin profile TEST est un test ELISA qui détecte simultanément cinq anticorps anti-peau spécifiques (anti-dsg1, anti-dsg3, anti-bp180, anti-bp230 et anti-collagène de type VII) dans le sérum du patient. Des puits de microtitration (8 puits) sont tapissés séparément avec 5 antigènes recombinants différents. Des anticorps spécifiques dans le sérum dilué se lieront au puits tapissé d'antigènes (incubation de l'échantillon). Les puits sont ensuite lavés pour retirer les anticorps non liés et les autres composants. Un conjugué d'anticorps anti-igg humaine marqué à la peroxydase de raifort est ajouté aux puits et incubé (incubation du conjugué). Après une nouvelle étape de lavage, le substrat peroxydase est ajouté et incubé une nouvelle fois (incubation du substrat). Une solution acide est ensuite ajoutée pour arrêter la réaction enzymatique et stabiliser le développement de la couleur. Le résultat de l'analyse est déterminé photométriquement en mesurant l'absorbance et en mettant les résultats en graphique. PROCÉDURE D'ANALYSE EN BREF <Incubation de l'échantillon> Ajouter 100 µl d'échantillon dilué (1:101) aux puits désignés de la Micro (20-30 C) 30 min. plaque selon le schéma Laver < Incubation du Conjugué > Ajouter 100 µl du Conjugué (prêt à l emploi) à chacun des puits (20-30 C) 30 min. Laver < Incubation du Substrat > Ajouter 100 µl de Substrat à chacun des puits (20-30 C) 15 min Ajouter 100 µl de la Solution d'arrêt à chacun des puits Lire l'absorbance Interpréter le résultat RÉACTIFS ET CONSERVATION 1) Micro plaques tapissés d'antigènes Micro plaques à 96 (12 x 8) puits tapissés de cinq antigènes recombinants différents. Les plaques sont emballées dans un porte-barrettes et scellées dans une pochette en aluminium contenant un dessiccateur. 2) Calibrateur 1 (0 U/mL) Un flacon contenant 1,5 ml du Diluant sérum contenant 0,09% d'azoture de sodium. Les composants aussi bien des trousses non ouvertes que des trousses ouvertes pour la première fois sont stables entre 2 et 8 C jusqu'à la date d'expiration mentionnée sur l'étiquette. PRÉCAUTIONS (1) Ce produit ne peut être utilisé que pour le diagnostic in vitro. (2) Ne pas utiliser les composants de la trousse après la date d'expiration. (3) Éviter le contact des réactifs avec les yeux, la peau ou les vêtements. En cas de contact des réactifs avec la peau, laver avec beaucoup d'eau. Le TMB est irritant et la Solution d'arrêt contient 0,25 mol/l d'acide sulfurique qui est un poison et corrosif. (4) Le Calibrateur 2 est un dérivé du sérum humain dans lequel l'antigène HBs, l'anticorps HCV, les anticorps HIV-1 et HIV-2 n'ont pas été détectés. Aucune méthode d'analyse ne peut cependant garantir l'absence de ceux-ci ni de tout autre agent infectieux. Ces réactifs et tous les échantillons de patients doivent être manipulés comme s'ils étaient capables de transmettre le SIDA, l'hépatite ou n'importe quelle autre maladie infectieuse. (5) Le Calibrateur 1, le Calibrateur 2 et le Diluant sérum contiennent de l'azoture de sodium (0,09%) comme agent conservateur et doivent être manipulés avec précaution. Ne pas ingérer ou mettre en contact avec la peau ou les muqueuses. L'azoture de sodium peut réagir avec le cuivre ou le plomb des tuyauteries et former des azotures métalliques explosifs. C'est pourquoi il faut toujours rincer abondamment à l'eau lorsque l'on jette dans les canalisations des substances contenant de l'azoture de sodium. (6) Le Calibrateur 1, le Calibrateur 2, le Conjugué (prêt à l emploi) et le Diluant sérum contiennent des substances d'origine animale prélevées sur des animaux non infectés. Ces composants doivent cependant être manipulés et jetés comme des dangers biologiques potentiels. (7) Ne pas mélanger avec des réactifs d'autres trousses. (8) Tous les réactifs doivent être portés à température ambiante (20-30 C) avant le lancement de l'essai. (9) Ne pas exposer la trousse aux rayons directs du soleil pendant l'analyse et lors du stockage. (10) Éviter la contamination microbienne et la contamination croisée des réactifs et des sérums. (11) Des températures d'incubation supérieures ou inférieures à une température ambiante normale (20-30 C), des temps d'incubation plus longs ou plus courts et des dilutions incorrectes peuvent affecter les résultats. 11

14 Français (12) Les puits doivent être correctement rincés avec la Solution de lavage pour éviter des résultats faussement positifs. (13) Pipeter soigneusement chacun des échantillons et chacun des réactifs pour éviter la contamination croisée entre les puits de microtitration. Éviter la formation de mousse. (14) Toutes les barrettes non utilisées doivent être remises dans la pochette à glissière qui, elle-même, doit être soigneusement rescellée pour éviter l'absorption d'humidité. (15) Le Solution de lavage (20x) peut devenir trouble entre 2 et 8 C. Cela n'affecte pas les résultats. (16) Les substances utilisées pour l'analyse doivent être jetées et traitées comme décrit ci-dessous. Faire tremper dans une solution de glutaraldéhyde à une concentration finale de 2% pendant plus d'une heure, faire tremper dans une solution d'hypochlorite de sodium à 0,1% (chlore disponible: approximativement ppm) pendant plus d'une heure ou autoclaver à 121 C pendant plus de 20 minutes. (17) Les valeurs d'anticorps obtenues par cet essai ne sont qu'une aide au diagnostic. Le médecin doit interpréter ces résultats à la lumière des antécédents du patient, de l'examen physique et d'autres procédures de diagnostic. (18) Si l'emballage protecteur est abîmé, les composants de la trousse doivent être traités ou jetés selon les instructions pertinentes et les règlementations de chacun des pays. Solution de lavage: préparer une dilution 1:20 de la Solution de lavage (20x) avant utilisation (par ex., ajouter 50 ml de Solution de lavage (20x) à 950 ml d'eau distillée). La Solution de lavage diluée est stable pendant 2 semaines entre 2 et 8 C. 4. Ne pas diluer le Calibrateur 1, le Calibrateur 2, le Conjugué (prêt à l emploi), le Diluant sérum, le Substrat et la Solution d'arrêt. Ces réactifs sont prêts à l'emploi. PRÉPARATION DES ÉCHANTILLONS 1. Utiliser des sérums de patients frais. Les échantillons doivent être testés le plus rapidement possible après leur prélèvement. S'il faut les conserver, les échantillons doivent être conservés à -20 C ou plus bas. Ne pas répéter la congélation et la décongélation. 2. Diluer chacun des sérums de patient à 1:101 en ajoutant 10 µl de sérum à 1 ml de Diluant séum. 12

ÉTAPE 3. (INCUBATION DU CONJUGUÉ) Ajouter 100 µl de Conjugué (prêt à l emploi) à chacun des puits avec une pipette multicanaux. Recouvrir les puits d'un couvercle de plaque de microtitration et incuber pendant 30 minutes à température ambiante (20-30 C). * Ne pas remettre le Conjugué (prêt à l emploi) dans le flacon une fois qu'il a été prélevé. ÉTAPE 4. (LAVAGE) Laver la plaque de microtitration comme décrit à l'étape 2. ÉTAPE 5. (INCUBATION DU SUBSTRAT) Ajouter 100 µl de Substrat à chacun des puits avec une pipette multicanaux. * Ne pas remettre le Substrat dans le flacon une fois qu'il a été prélevé. Si l'on utilise un lecteur de plaques à deux longueurs d'onde, régler la longueur d'onde du test à 450 nm et la référence à 620 nm. * La lecture doit se faire dans les 20 minutes après la réaction d'arrêt. * Avant de lire la plaque, s'assurer que le fond de la plaque est propre et sec et qu'il n'y a pas de bulles d'air à la surface du liquide dans les puits. 19 Deutsch Deutsch VERWENDUNGSZWECK Der MESACUP anti-skin profile TEST ist ein qualitatives enzyme-linked immunosorbent assay (ELISA) Testsystem zum Nachweis von IgG Antikörpern gegen Dsg1, Dsg3, BP180, BP230 und TypeVII Kollagen im menschlichen Serum. Fünf individuelle Autoantikörper können in einem einzigen Test gleichzeitig nachgewiesen werden. Der MESACUP anti-skin profile TEST ist zum in vitro Gebrauch vorgesehen, als Hilfe bei der Diagnose bestimmter blasenbildender Autoimmunerkrankungen. ZUSAMMENFASSUNG UND KLINISCHER HINTERGRUND Autoimmune blasenbildende Erkrankungen sind eine Gruppe von Hauterkrankungen, die durch Blasenbildung oder Erosionen der Haut gekennzeichnet und als pemphigus und pemphigoid klassifiziert sind. Pemphigus ist eine autoimmune blasige Hauterkrankung, die die Haut und muköse Membranen zum Ziel hat. Es werden zwei Hauptuntertypen unterschieden: Pemphigus vulgaris (PV) und Pemphigus foliaceus (PF). Bei Patienten mit PV werden Anti-Desmoglein 3 (Dsg3) Antikörper gebildet. Bei PV Patienten weist muco-dominante PV nur anti-dsg3 und mucocutane PV sowohl Dsg1 als auch Dsg3 Antikörper auf, wohingegen Anti Dsg1 positive aber Anti-Dsg3 negative Patienten als PF (1) diagnostiziert werden. Diese Anti-Dsg Antikörper bestehen aus pathogenen und nichtpathogenen Antikörpern. Eine Aktuelle Studie hat gezeigt, dass die pathogenen Anti-Dsg Antikörper an reife Dsg-Antigene binden, die unmaskiert durch proteolytische Prozesse unreifer Dsg (2) sind. Blasenbildende Pemphigoid (BP) ist eine subepidermale blasige Erkrankung, die durch die Präsenz von IgG-Autoantikörpern gegen epidermale Basismembranbereiche im Serum gekennzeichnet ist. Die Autoantikörper von Patienten mit BP zielen auf zwei ausgeprägte Antigene, ein 180 kda transmembranes Protein bezeichnet als BP180 und ein 230 kda intrazelluläres Protein bezeichnet als BP230. Epitopstudien von Anti-BP180 Antikörpern zeigten, dass die nicht-kollagene 16a (NC16a) Domäne von BP180 das Hauptepitop ist, welches von der der Mehrheit der BP Seren (3-4) erkannt wird. Bei Anti-BP230 Antikörpern reagiert die C-terminale Domäne (BP230-C) hauptsächlich mit BP Patientenserum, aber einige BP Patientenseren erkennen nur die N-terminale Domäne von BP230 (BP230-N) (5-6). Epidermolysis bullosa aquisita (EBA) ist eine blasenbildende Autoimmun-Hauterkrankung, die durch das Vorhandensein von Typ VII Kollagen IgG-Autoantikörpern gekennzeichnet ist. Typ VII Kollagen ist aus drei identischen α-ketten zusammengesetzt. Jede α-kette besteht aus einer zentralen kollagenösen Domäne (COL), flankiert von einer 145 kda nicht-kollagenösen Amino-terminalen Domäne (NC1) und einer kleineren 34 kda Carboxyl-terminalen nicht-kollagenen Domäne (NC2). Die antigenen Hauptepitope für Anti-Typ VII kollagene Autoantikörper bei Patienten mit EBA befinden sich in der NC1 Domäne. Allerdings zeigt die aktuelle Forschungsveröffentlichung, dass die NC2 Domäne für diese Autoantikörper ein weniger wichtiges antigenes Epitop darstellt (7). In der folgenden Tabelle sind die, in diesem Profil-Kit verwendeten, rekombinanten Antigene aufgelistet, die den oben besprochenen Hintergrund aufweisen. 20 Deutsch WIRKUNGSPRINZIP Der MESACUP anti-skin profile TEST ist ein ELISA, der fünf Anti-Haut-spezifische Antikörper (anti-dsg1, Anti-Dsg3, Anti-BP180, Anti-BP230 und Anti-TypVII Kollagen) gleichzeitig im Patientenserum entdeckt. Mikrotiterstreifen (8 Vertiefungen) sind separat mit 5 verschiedenen rekombinanten Antigenen beschichtet. Spezifische Antikörper im verdünnten Serum binden an die Antigen beschichtete Vertiefung (Probeninkubation). Die Vertiefungen werden dann gewaschen, um ungebundene Antikörper und andere Komponenten zu entfernen. Ein Konjugat von Meerettich Peroxidase markiertem Anti-human IgG Antikörper wird zu jeder Vertiefung hinzugegeben und für eine weitere Zeitspanne inkubiert (Substratinkubation). Säurelösung wird danach zu jaeder Vertiefung gegeben, um die Enzymreaktion zu stoppen und die Farbentwicklung zu stabilisieren. Das Testergebnis wird photometrisch durch Messung der Absorbtion und Aufzeichnung der Ergebnisse bestimmt. (4) Kalibrator 2 stammt aus menschlichem Serum, in dem keine HBs Antigene und Antikörper gegen HCV, HIV-1 und HIV-2 nachgewiesen wurden. Keine Testmethode kann jedoch die Abwesenheit dieser oder anderer infektiöser Agenzien garantieren. Diese Reagenzien und alle Patientenproben sollten behandelt werden, als ob sie AIDS, Hepatitis oder andere infektiöse Erkrankungen übertragen könnten. (5) Kalibrator 1, Kalibrator 2 und der Probendiluent enthalten Natriumazid (0,09%) als Konservierungsmittel und müssen mit Vorsicht behandelt werden.nicht verschlucken und Kontakt mit Haut oder mukösen Membranen vermeiden. Natriumazid kann mit Kupfer oder Blei zu explosiven Metallaziden reagieren. Spülen Sie deshalb mit viel Wasser, wenn Sie Natriumazid haltiges Material in der Spüle entsorgen. (6) Kalibrator 1, Kalibrator 2, Konjugatreagenz und Probendiluent enthalten Material tierischen Ursprungs, gewonnen aus nicht-infizierten Tieren. Diese Komponenten sollten trotzdem im Gebrauch und bei der Entsorgung als potentiell biogefährdend betrachtet werden. (7) Mischen Sie keine Reagenzien von anderen Testsystemen. (8) Alle Reagenzien müssen vor Testbeginn auf Raumtemperatur (20-30 C) gebracht werden. (9) Setzen Sie das Testsystem während des Tests und der Lagerung nicht direktem Sonnenlicht aus. (10) Vermeiden Sie mikrobielle und Kreuzkontaminationen der Reagenzien und Proben. (11) Inkubationstemperaturen ober- oder unterhalb normaler Raumtemperatur (20-30 C), kürzere oder längere Inkubationszeiten und unkorrekte Verdünnungen können zu verkehrten Ergebnissen führen. (12) Die Vertiefungen müssen gut mit Waschlösung gespült werden, um falsche Ergebnisse zu vermeiden. (13) Pipettieren Sie jede Probe und Reagenz sorgfältig, um Kreuzkontaminationen zwischen den Microwells 19

22 Deutsch zu vermeiden. Vermeiden Sie Schaumbildung. (14) Alle unbenutzten Mikrotiterstreifen sollten in die Ziplock Tasche zurückgesteckt werden. Diese muss sorgfältig versiegelt werden, um Absorption von Feuchtigkeit zu vermeiden. (15) Waschkonzentrat kann bei 2-8 C trübe werden. Dieses beeinflußt die Ergebnisse nicht. (16) Die für den Test benutzten Materialien sollten wie nachfolgend beschrieben entsorgt oder behandelt werden. Weichen Sie das Material mehr als 1 Stunde in 2% iger (Endkonzentration) Glutaraldehydlösung ein, mehr als 1 Stunde Einweichen in 0,1%iger Natriumhypochloridlösung (verfügbares Chlor ca ppm) oder bei 121 C für mehr als 20 Minuten Autoklavieren. (17) Die aus diesem Test erhaltenen Antikörperwerte stellen nur eine diagnostische Hilfe dar. Jeder Arzt muss diese Ergebnisse im Licht der Patientenhistorie, der physischen Befunde und anderer diagnostischer Verfahren bewerten. (18) Ist die Schutzverpackung beschädigt, sollten die Testsystemkomponenten entsprechend den geltenden Vorschriften des entsprechenden Landes behandelt oder entsorgt werden. ZUSÄTZLICH BENÖTIGTES MATERIAL Mikroplattenleser (Wellenlänge: 450 nm, 620 nm/referenz) Multikanal Mikropipette (z.b. 100 µl µl) Einkanalpipette (z.b. 10 µl & 100 µl) Automatischer Wascher oder Waschflasche Deionisiertes oder distilliertes Wasser Zylinder (1 Liter, mit Graduierung) für die Zubereitung der Waschlösung Teströhrchen für die Verdünnung von Patientenproben (z.b µl) Einmal Pipettenspitzen Papiertücher Mikroplattenabdeckung TESTABLAUF VORBEREITUNG DER REAGENZIEN 1. Bringen Sie vor der Anwendung alle Testsystem Materialien auf Raumtemperatur (20-30 C). 2. Mikrotiterstreifen: Jeweils ein Streifen muss pro Patientenserum verwendet werden. Entnehmen Sie die benötigte Anzahl Mikrotiterstreifen aus der Tasche und setzen Sie sie in den Halterahmen ein. Die unbenutzten Streifen müssen unverzüglich wieder im Kühlschrank gelagert werden. 3. Waschlösung: Bereiten Sie eine 1:20 Verdünnung des Waschkonzentrats vor der Verwendung zu (fügen Sie 50 ml Waschkonzentrat zu 950 ml destilliertem Wasser). Die verdünnte Waschlösung bleibt bei 2-8 C 2 Wochen stabil. 4. Verdünnen Sie nicht Kalibrator 1, Kalibrator 2, das Konjugatreagenz, die Probendiluent, das Substrat und die Stoplösung. Diese Reagenzien sind gebrauchsfertig. VORBEREITUNG DER PROBEN 1. Benutzen Sie frisches Patientenserum. Die Proben sollten so bald wie möglich nach Entnahme getestet werden. Ist Lagerung notwendig, sollten die Proben bei -20 C oder niedriger gelagert werden. Wiederholen Sie keine Einfrier-und Auftauvorgänge. 2. Verdünnen Sie jedes Patientenserum 1:101 durch hinzufügen von 10 µl Serum zu 1 ml 20

23 Deutsch Probendiluent. * Verdünnte Proben können nicht aufbewahrt werden. Verdünnen Sie jedes Patientenserum für jede Testsystem separat. TESTPROZEDUR * Die Probendiluent kann Präzipitate bilden, die die Testergebnisse nicht beeinflussen. SCHRITT 1. (PROBENINKUBATION) Mit einer Multikanalpipette werden jeweils 100 µl Kalibrator 1, Kalibrator 2, verdünntes Patientenserum und Probendiluent in jede festgelegte Vertiefung der Mikrotiterplatte entsprechend dem untenstehenden Schema gegeben. Das Pipettieren sollte so schnell wie möglich vollendet werden. Bedecken Sie die Platte mit einer Mikroplattenabdeckung und inkubieren Sie sie bei Raumtemperatur (20-30 C) für 30 Min. SCHRITT 2. (WASCHEN) Saugen oder verwerfen Sie die Inhalte der Vertiefungen. Füllen Sie die Vertiefung mit Waschlösung und saugen Sie sie komplett ab. Waschen Sie 4 mal. Klopfen Sie die Platte auf ein Papiertuch und entfernen Sie die verbliebene Waschlösung. Wenn ein automatischer Wascher benutzt wird, waschen Sie 4 mal. * Jedes Labor muss seine geeigneten eigenen Waschzeiten und anderen Bedingungen verifizieren. * Die Waschlösung sollte bei C benutzt werden. SCHRITT 3. (KONJUGATINKUBATION) Fügen Sie 100 µl Konjugatreagenz mit einer Multikanalpipette zu jeder Vertiefung. Bedecken Sie die Platte mit einer Mikroplattenabdeckung und inkubieren Sie sie bei Raumtemperatur (20-30 C) für 30 Min. * Das aus der Flasche entnommene Konjugatreagenz nicht wieder zurückpipettieren. SCHRITT 4. (WASCHEN) Waschen Sie die Mikrotiterplatte, wie in Schritt 2 beschrieben. SCHRITT 5. (SUBSTRATINKUBATION) Geben Sie 100 µl des Substrats mit einer Multikanalpipette in jede Vertiefung. * Geben Sie das entnommene Substrat nicht wieder in das Fläschen zurück Bedecken Sie die Vertiefungen mit einer Mikroplattenabdeckung und inkubieren Sie sie bei Raumtemperatur (20-30 C) für 15 Min. 21
