1 PLATELIA CMV IgG TESTS QUANTITATIVE DETERMINATION OF IgG ANTIBODIES TO CYTOMEGALOVIRUS IN HUMAN SERUM OR PLASMA BY ENZYME IMMUNOASSAY

2 1. INTENDED USE Platelia CMV IgG is an indirect ELISA immunoassay for quantitative determination of IgG antibodies to cytomegalovirus in human serum or plasma. 2. CLINICAL VALUE The human cytomegalovirus (CMV) is a ubiquitous virus found in all geographical areas in the world and in all socio-economical groups. The CMV is a member of the Herpesvirus family and is transmitted through biological fluids during close interpersonal contacts (urine, saliva, maternal milk, blood, tears, sperm and vaginal secretions). After an infection, the CMV can remain latent during several years in body of contaminated patients, and then can reactivate causing recurrent infections with potential transmission to other individuals. Primary infection is acquired by different ways of transmission and at different periods of life (congenital infections, post-natal infections). After latency phase, the secondary infection may occur either through re-infection by an exogenous virus or by reactivation of the latent virus. 50 to 85% of the population is infected before adult age, but most of the infections remain asymptomatic, with only 2% of the patients showing atypical symptoms like fever, asthenia, muscles or joint pains. However, the CMV infection can be serious when affecting pregnant women, neonates or immunocompromised hosts. During pregnancy, 1 to 3 % of women are infected with CMV and fetal transmission through placental diffusion is observed in 40 to 50% of patients. 95% of fetal infections are not symptomatic, but in 5%, complications are extremely severe: hepatosplenomegaly, microcephaly, hydrocephaly, prematurity, psychomotor retardation and fetal death. In 10% of asymptomatic infections, neonates will present delayed complications like partial or complete deafness or blindness. In immunocompromised patients (AIDS, organ transplant recipients, lymphoproliferative diseases or cancer), severe complications are related to a disseminated and/or visceral infection: splenomegaly, chrorioretinitis, hepatitis, pneumonia, myocarditis, encephalitis. The infection can be fatal in these patients. A few weeks after the CMV infection, the immune response leads to the appearance of specific IgM antibodies, followed a few days later by the appearance of specific IgG antibodies. The serological diagnosis of CMV infection is demonstrated by a seroconversion or a significant increase with titer of IgG. Detection of specific anti-cmv IgG antibodies is useful for the diagnosis of a primary infection and to determine the immune status of patients. 2

3 3. PRINCIPLE Platelia CMV IgG is a test for detection and titration of IgG antibodies to Cytomegalovirus in human serum or plasma using an indirect ELISA immunoenzymatic method. Inactivated CMV antigen is used for coating the microplate. A polyclonal antibody labeled with peroxydase which is specific for human gamma chains (anti-igg) is used as the conjugate. The test uses the following steps: Step 1 Patients samples and calibrators are diluted 1/21 and then distributed in the wells of the microplate. During this incubation of one hour at 37 C, IgG antibodies to CMV present in the sample bind to the CMV antigen coated on microplate wells. After incubation, unbound non specific antibodies and other serum proteins are removed by washings. Step 2 The conjugate (peroxydase labeled polyclonal antibody specific for human gamma chains) is added to the microplate wells. During this incubation of one hour at 37 C, the labeled antibody binds to the serum IgG captured by the CMV antigen. The unbound conjugate is removed by washings at the end of the incubation. Step 3 The presence of immune-complexes (CMV antigen, IgG antibodies to CMV, anti-igg conjugate) is demonstrated by the addition in each well of an enzymatic development solution. Step 4 After incubation at room temperature ( C), the enzymatic reaction is stopped by addition of 1N sulfuric acid solution. The optical density reading obtained with a spectrophotometer set at 450/620 nm is proportional to the amount of IgG antibodies to CMV present in the sample. 3

WARNINGS AND PRECAUTIONS The reliability of the results depends on correct implementation of the following Good Laboratory Practices: Do not use expired reagents. Do not mix or associate within a given run reagents from different lots. REMARK: For Washing Solution (R2, label identification: 20x colored green), Chromogen (R9, label identification: TMB colored turquoise) and Stopping Solution (R10, label identification: 1N colored red), it is possible to use other lots than those contained in the kit, provided these reagents are strictly equivalent and the same lot is used within a given test run. REMARK: In addition, the Washing Solution (R2, label identification: 20x colored green) can be mixed with the 2 other washing solutions included in various Bio-Rad reagent kits (R2, label identifications: 10x colored blue or 10x colored orange) when properly reconstituted, provided only one mixture is used within a given test run. Before use, wait for 30 minutes to allow reagents to reach room temperature ( C). Carefully reconstitute or dilute the reagents avoiding any contamination. Do not carry out the test in the presence of reactive vapors (acid, alkaline, aldehyde vapors) or dust that could alter the enzyme activity of the conjugate. Use glassware thoroughly washed and rinsed with deionized water or, preferably disposable material. 5

6 Washing the microplate is a critical step in the procedure: follow the recommended number of washings cycles and make sure that all wells are completely filled and then completely emptied. Incorrect washings may lead to inaccurate results. Do not allow the microplate to dry between the end of the washings operation and the reagent distribution. Never use the same container to distribute the conjugate and the development solution. The enzymatic reaction is very sensitive to metal or metal ions. Consequently, do not allow any metal element to come into contact with the various solutions containing the conjugate or the chromogen. Chromogen solution (R9) should be colorless. The appearance of a blue color indicates that the reagent cannot be used and must be replaced. Use a new pipette tip for each sample. Check the pipettes and other equipments for accuracy and correct operations. 6 HEALTH AND SAFETY INSTRUCTIONS Human origin material used in the preparation of reagents has been tested and found non-reactive for hepatitis B surface antigen (HBs Ag), antibodies for hepatitis C virus (anti-hcv), and to human immunodeficiency virus (anti-hiv1 and anti-hiv2). Because no method can absolutely guarantee the absence of infectious agents, handle reagents of human origin and patient samples as potentially capable of transmitting infectious diseases: Any material, including washings solutions, that comes directly in contact with samples and reagents containing materials of human origin should be considered capable of transmitting infectious diseases. Wear disposable gloves when handling samples and reagents. Do not pipette by mouth. Avoid spilling samples or solutions containing samples. Spills must be rinsed with bleach diluted to 10 %. In the event of a spill with an acid, it must be first neutralized with sodium bicarbonate, and then cleaned with bleach diluted to 10% and dried with adsorbent paper. The material used for cleaning must be discarded in a contaminated residue container. Patient samples, reagents containing human origin material, as well as contaminated material and products should be discarded after decontamination only: - either by immersion in bleach at the final concentration of 5 % of sodium hypochloride during 30 minutes, - or by autoclaving at 121 C for 2 hours at the minimum. CAUTION: Do not introduce solutions containing sodium hypochloride into the autoclave

7 Avoid any contact of reagents, including those considered as not dangerous, with skin and mucosa. Chemical and biological residues must be handled and disposed off in accordance with Good Laboratories Practices. All reagents in the kit are exclusively for in vitro diagnostic use. Caution: Some of the reagents contain ProClin 300 < 1.5% R43: May cause sensitisation by skin contact S28-37: After contact with skin, wash immediately with plenty of Xi - Irritant water and soap. Wear suitable gloves 6. SAMPLES 1. Serum and plasma (EDTA, heparin or citrate) are the recommended sample types. 2. Observe the following recommendations for handling, processing and storage of blood samples: Collect all blood samples observing routine precaution for venipuncture. For serum, allow samples to clot completely before centrifugation. Keep tubes stoppered at all times. After centrifugation, separate the serum or plasma from the clot or red cells in a tightly stoppered storage tube. The specimens can be stored at +2-8 C if test is performed within 7 days. If test will not be completed within 7 days, or for shipment, freeze the samples at -20 C or colder. Do not use samples that have been thawed more than five times. Previously frozen specimens should be thoroughly mixed (Vortex) after thawing prior to testing. 3. Samples containing 90 g/l of albumin or 100 mg/l of unconjugated bilirubin, lipemic samples containing the equivalent of 36 g/l of triolein (triglyceride), and hemolysed samples containing up to 10 g/l of hemoglobin do not affect the results. 4. Do not heat the samples. 7. ASSAY PROCEDURE 7.1 Materials required but not provided Vortex mixer. Microplate reader equipped with 450 nm and 620 nm filters (*). Microplate incubator thermostatically set at 37±1 C (*). Automatic, semi-automatic or manual microplate washer (*). Sterile distilled or deionized water. Disposable gloves. 7

8 Goggles or safety glasses. Adsorbent paper. Automatic or semi-automatic, adjustable or preset, pipettes or multipipettes, to measure and dispense 10 µl to 1000 µl, and 1 ml, 2 ml and 10 ml. Graduated cylinders of 25 ml, 50 ml, 100 ml and 1000 ml capacity. Sodium hypochloride (bleach) and sodium bicarbonate. Container for biohazard waste. Disposable tubes. (*) Consult our technical department for detailed information about the recommended equipment. 7.2 Reagents reconstitution R1: Allow 30 minutes at room temperature ( C) before opening the bag. Take out the carrier tray, return unused strips in the bag immediately and check the presence of desiccant. Carefully reseal the bag and store it at +2-8 C. R2: Dilute 1/20 the washing solution R2 in distilled water: for example 50 ml of R2 and 950 ml of distilled water to get the ready-to-use washing solution. Prepare 350 ml of diluted washing solution for one plate of 12 strips if washing manually. R3, R4a, R4b, R4c, R4d: Dilute 1/21 in Diluent (R7) (example: 300 µl of R µl of Calibrator). R6+R7: Conjugate (R6) is concentrated 51x and must be homogenize before use. Dilute 1/51 in Diluent (R7). For one plate, dilute 0.5 ml of Conjugate (R6) in 25 ml of Diluent (R7). Divide these volumes by 10 to obtain the volume needed for one strip. 7.3 Storage and validity of opened and / or reconstituted reagents The kit must be stored at +2-8 C. When the kit is stored at +2-8 C before opening, each component can be used until the expiration date indicated on the outer label of the kit. R1: Once opened, the strips remain stable for up to 8 weeks if stored at +2-8 C in the same carefully closed bag (check the presence of desiccant). R2: Once diluted, the Washing Solution can be kept for 2 weeks at C. Once opened, the concentrated Washing Solution stored at C, in absence of contamination, is stable until the expiration date indicated on the label. R3, R4a, R4b, R4c, R4d, R6, R7: Once opened and without any contamination, the reagents stored at +2-8 C are stable for up to 8 weeks. R6+R7: Once diluted, the conjugate working solution is stable for 8 hours at room temperature ( C) or 24 hours at +2-8 C. 8

9 R9: Once opened and without any contamination, the reagent stored at +2-8 C is stable for up to 8 weeks. R10: Once opened and without any contamination, the reagent stored at +2-8 C is stable until the expiration date indicated on the label. 7.4 Procedure Strictly follow the assay procedure and Good Laboratory Practices. Before use, allow reagents to reach room temperature ( C). The use of breakable wells requires a special attention during handling. Use all calibrators with each run to validate the assay results. 1. Carefully establish the distribution and identification plan for calibrators and patients samples. 2. Prepare the diluted Washing Solution (R2) [Refer to Section 7.2]. 3. Take the carrier tray and the strips (R1) out of the protective pouch [Refer to Section 7.2]. 4. In individually identified tubes, dilute Calibrators R3, R4a, R4b, R4c and R4d and patients samples (S1, S2 ) in Diluent (R7) to give a 1/21 dilution: 300 µl of Diluent (R7) and 15 µl of sample [Refer to Section 7.2]. Vortex diluted samples. 5. Strictly following the indicated sequence below, distribute in each well with 200µl of diluted calibrators and patient samples: A R3 S4 S12 B R4a S5 C R4b S6 D R4c S7 E R4d S8 F S1 S9 G S2 S10 H S3 S11 6. Cover the microplate with an adhesive plate sealer, then press firmly onto the plate to ensure a tight seal. Incubate the microplate immediately in a thermostat controlled water bath or in a dry incubator for 1 hour ± 5 minutes at 37 C ± 1 C. 7. Before the end of the first incubation period, prepare the conjugate working solution (R6+R7) [Refer to Section 7.2]. 8. At the end of the first incubation period, remove the adhesive plate sealer. Aspirate the content of all wells into a container for biohazard waste (containing sodium hypochloride). Wash microplate 4 times with 350 µl of 9

10 the Washing Solution (R2). Invert the microplate and gently tap on adsorbent paper to remove remaining liquid. 9. Distribute immediately 200 µl of the conjugate working solution (R6+R7) in all wells. The solution must be shaken gently before use. 10. Cover the microplate with an adhesive plate sealer, then press firmly onto the plate to ensure a tight seal. Incubate the microplate immediately in a thermostat controlled water bath or in a dry incubator for 1 hour ± 5 minutes at 37 C ± 1 C. 11. At the end of the second incubation period, remove the adhesive plate sealer. Aspirate the content of all wells into a container for biohazard waste (containing sodium hypochloride). Wash microplate 4 times with 350 µl of the Washing Solution (R2). Invert the microplate and gently tap on adsorbent paper to remove remaining liquid. 12. Quickly distribute into each well and away from light 200 µl of Chromogen solution (R9). Allow the reaction to develop in the dark for 30 ± 5 minutes at room temperature ( C). Do no use adhesive plate sealer during this incubation. 13. Stop the enzymatic reaction by adding 100 µl of Stopping Solution (R10) in each well. Use the same sequence and rate of distribution as for the development solution. 14. Carefully wipe the plate bottom. Read the optical density at 450/620 nm using a plate reader within 30 minutes after stopping the reaction. The strips must always be kept away from light before reading. 15. Before reporting results, check for agreement between the reading and the distribution plan of plate and samples. 8. INTERPRETATION OF RESULTS 8.1 Establishing the standard curve There is no international standard preparation available for measurement of anti-cmv IgG antibodies. The Platelia CMV IgG assay is standardized against the Proposed International Standard Preparation WHO Presence and concentration of IgG antibodies to CMV in a sample will be determined by comparison of optical density (OD) of this sample to the concentration in Arbitrary Units per milliliter (AU/ml) of the calibrators of the standard curve. Draw the standard curve [OD = function (AU/ml)] by plotting OD readings of calibrators R3, R4a, R4b, R4c and R4d on the vertical (Y) axis, then by plotting their respective concentration in AU/ml on the horizontal (X) axis. For each tested sample, calculate the anti-cmv IgG antibody titer by determining from the drawn standard curve the corresponding concentration to the measured OD. 10

11 8.2 Quality Control Include all calibrators for each microplate and for each run, and analyze the obtained results. For validation of the assay, the following criteria must be met: Optical density values: OD R4a OD R4b OD R4d > OD R4c Optical density ratios: OD R4a / OD R OD R4b / OD R4a 1.50 OD R4c / OD R4b 1.50 If those quality control criteria are not met, the test run should be repeated. 8.3 Interpretation of results Anti-CMV antibodies titer (Arbitrary Units/ml) Result Interpretation Titer < 0.25 AU/ml Negative A negative or equivocal result is indicative of absence of acquired immunity, but cannot 0.25 AU/ml Titer < 0.5 AU/ml Equivocal exclude a recent infection. If a recent infection of the patient is suspected, a second sample should be run about two weeks later. Titer 0.5 AU/ml Positive A positive result is usually indicative of a pastinfection. However, a recent infection cannot be excluded, especially if anti-cmv IgM antibodies are present. If a recent infection is suspected, other serological tests like measurement of IgG antibodies avidity can be useful to confirm the diagnosis. A higher variation of titers can be observed above concentrations of 4 AU/ml. Samples giving concentrations above the higher calibrator of the standard curve can be diluted in Diluent R7 in order to obtain a result within the assay linearity range. In such situation, the final concentration is obtained by multiplying the measured result by the dilution rate. 8.4 Trouble Shooting Guide Non validated or non repeatable reactions are often caused by: Inadequate microplate washings. Contamination of negative samples by serum or plasma with a high antibody titer. Contamination of the development solution by chemical oxidizing agents (bleach, metal ions...). Contamination of the Stopping Solution. 11

PERFORMANCES Performances of Platelia CMV IgG were evaluated at 2 sites using a total of 1096 samples from pregnant women and blood donors. 9.1 Prevalence Prevalence of anti-cmv IgG antibodies using Platelia CMV IgG was estimated on a panel of 300 samples from pregnant women of Paris area (France). 145 samples were positive for anti-cmv IgG antibodies. Prevalence measured with Platelia CMV IgG assay is established at 48.3% (145/300). 9.2 Specificity Relative specificity was estimated using a panel of 490 samples from 2 sites located in France and split as follows: 61 sera from blood donors 429 sera from pregnant women Results were compared with those obtained using a commercialized EIA assay considered as a reference. [IC 95%] = 95% confidence interval. 9.4 Cross Reactivity A panel of 103 samples including 77 positive samples for Toxoplasmosis, Rubella, EBV, HSV, VZV, mumps, measles and HIV and 26 positive samples for rheumatoid factor, auto-antibodies and heterophile antibodies were tested with Platelia CMV IgG and a commercialized EIA assay for screening of anti-cmv IgG antibodies. Among theses samples, 37 were found negative and concordant with the EIA kit used for comparison, confirming the absence of potential cross-reactions. 9.5 Precision Within-run precision (repeatability): In order to evaluate intra-assay repeatability, one negative and three positive samples were tested 30 times during the same run. The concentration (AU/ml) was determined for each positive sample. Results for the negative sample are expressed in ratio. 14 Between-run precision (reproducibility) : In order to evaluate inter-assay reproducibility, the four samples (one negative and three positive) were tested in duplicate in two runs per day over a 20 days period. The concentration (AU/ml) was determined for each positive sample. Results for the negative sample are expressed in ratio. LIMITATIONS OF THE PROCEDURE Diagnosis of CMV infection can only be established on the basis of a combination of clinical and biological data. The result of a single test of titration of anti-cmv IgG antibodies does not constitute sufficient proof for the diagnosis of a recent infection by Cytomegalovirus. 11. QUALITY CONTROL OF THE MANUFACTURER All manufactured reagents are prepared according to our Quality System, starting from reception of raw material to commercialization of the final product. Each lot is submitted to quality control assessments and is released to the market only after conforming to pre-defined acceptance criteria. DOMAINE D UTILISATION Platelia CMV IgG est un test immunoenzymatique de type ELISA indirect pour la détermination quantitative des anticorps IgG dirigés contre le cytomégalovirus dans le sérum ou le plasma humain. 2. INTERET CLINIQUE Le cytomégalovirus humain (CMV) est un virus ubiquitaire retrouvé dans toutes les zones géographiques du globe et dans tous les groupes socioéconomiques. Le CMV est un membre de la famille des Herpesvirus et se transmet par l intermédiaire des liquides biologiques lors de contacts humains étroits (urine, salive, lait maternel, sang, larmes, liquide séminal et secrétions vaginales). Après infection, le CMV persiste pendant des années dans l organisme des sujets atteints, et peut être à l origine d infections récurrentes et être transmis à d autres individus. L infection primaire s acquière selon différents modes de transmission et à différentes périodes de la vie (infections congénitales, infections post-natales). Après la période de latence, l infection secondaire peut survenir soit par réinfection par un virus exogène, soit par réactivation du virus latent. Entre 50 et 85% de la population est infectée avant l âge adulte, mais la plupart des infections restent asymptomatiques, et seulement 2% des patients présentent des signes atypiques tels que fièvre, asthénie, douleurs musculaires ou articulaires. Cependant, l infection par le CMV peut être très grave lorsqu elle intervient chez une femme enceinte, un nouveau-né ou un patient immunodéprimé. Au cours de la grossesse, 1 à 3 % des femmes sont infectées par le CMV et la transmission au fœtus par passage transplacentaire est observée dans 40 à 50% des cas. 95% des infections fœtales sont asymptomatiques, mais dans 5% des cas les complications sont extrêmement sévères : hépatosplénomégalie, microcéphalie, hydrocéphalie, prématurité, retard psychomoteur, mort fœtale. Dans 10% des infections asymptomatiques, les nouveaux nés présenteront des complications retardées telles qu une surdité ou une cécité partielles ou totales. Chez les sujets immunodéprimés (infection VIH, transplantation d organe, maladie lymphoproliférative ou cancer), des complications sévères surviennent également liées à une infection disséminée et/ou viscérale : splénomégalie, chroriorétinite, hépatite, pneumonie, myocardite, encéphalite. Chez ces patients, la maladie peut être fatale. Quelques semaines après l infection, la réponse immunitaire au CMV se traduit par l apparition d anticorps spécifiques de type IgM, suivie quelques jours plus tard par celle d anticorps de type IgG. Le diagnostic sérologique de l infection par le CMV est basé sur la mise en évidence d une séroconversion ou d une élévation significative du titre des anticorps IgG. La détection des anticorps IgG spécifiques du CMV est utile pour le diagnostic d une infection primaire et pour préciser le statut immunitaire des patients. 18

19 3. PRINCIPE Platelia CMV IgG est un test permettant la détection et le titrage des anticorps IgG anti-cmv dans le sérum ou le plasma humain par une méthode immunoenzymatique sur phase solide dite technique «ELISA indirect». L antigène CMV inactivé est utilisé pour sensibiliser la microplaque. Un anticorps polyclonal marqué à la peroxydase et spécifiquement dirigé contre les chaînes gamma humaines (anti-igg) est utilisé comme conjugué. La mise en œuvre du test comprend les étapes suivantes : Etape 1 Les échantillons à étudier ainsi que les calibrateurs sont dilués au 1/21 puis déposés dans les cupules de la microplaque. Durant cette incubation de 1 heure à 37 C, les IgG anti-cmv présentes dans l échantillon se lient à l antigène CMV fixé sur les cupules de la microplaque. Les IgG non spécifiques du CMV et les autres protéines sériques sont éliminées par les lavages pratiqués à la fin de l incubation. Etape 2 Le conjugué (anticorps polyclonal spécifique des chaînes gamma humaines et marqué à la peroxydase) est déposé dans toutes les cupules de la microplaque. Durant cette incubation de 1 heure à 37 C, l anticorps marqué se lie aux IgG sériques ayant réagi avec l antigène CMV. Le conjugué non lié est éliminé par les lavages pratiqués à la fin de l incubation. Etape 3 La présence des complexes (Ag CMV, IgG anti-cmv, conjugué anti-igg) éventuellement formés est révélée par l addition dans chaque cupule d une solution de révélation enzymatique. Etape 4 Après incubation à température ambiante ( C), la réaction enzymatique est stoppée par addition d une solution d acide sulfurique 1N. La densité optique lue à 450/620 nm est proportionnelle à la quantité d IgG anti-cmv présente dans l échantillon testé. 19

20 4. COMPOSITION DE LA TROUSSE Les réactifs sont fournis en quantité suffisante pour réaliser 96 déterminations. Tous les réactifs sont destinés à l usage exclusif du diagnostic in vitro. PRECAUTIONS D UTILISATION La qualité des résultats dépend du respect des Bonnes Pratiques de Laboratoire suivantes : Ne pas utiliser les réactifs après la date d'expiration. Ne pas mélanger, ni associer au cours d une même série, des réactifs provenant de trousses portant des numéros de lots différents. REMARQUE : Il est possible d utiliser d autres lots de Solution de Lavage (R2 identifié 20x en vert), de Chromogène (R9 identifié TMB en turquoise) et de Solution d Arrêt (R10 identifié 1N en rouge) que ceux fournis avec la trousse, sous réserve d utiliser des réactifs strictement équivalents d un seul et même lot au cours d une même série. REMARQUE : De plus, la Solution de Lavage (R2 identifié 20x en vert) peut être mélangée avec l une des deux autres Solutions de Lavage inclues dans les différents kits réactifs Bio-Rad (R2 identifié 10x en bleu ou 10x en orange) et correctement reconstituées, à condition qu un seul mélange soit utilisé au cours d une même série. Avant utilisation, attendre 30 minutes que les réactifs s équilibrent à la température ambiante ( C). Reconstituer ou diluer soigneusement les réactifs en évitant toute contamination. Ne pas réaliser le test en présence de vapeurs réactives (acides, alcalines, aldéhydes) ou de poussières qui pourraient altérer l activité enzymatique du conjugué. Utiliser de préférence du matériel à usage unique. A défaut, utiliser une verrerie parfaitement lavée et rincée à l eau distillée. 21

22 Le lavage des cupules est une étape essentielle de la manipulation : respecter le nombre de cycles de lavages prescrit, et s assurer que toutes les cupules sont complètement remplies, puis complètement vidées. Un mauvais lavage peut entraîner des résultats incorrects. Ne pas laisser la microplaque sécher entre la fin des lavages et la distribution des réactifs. Ne jamais utiliser le même récipient pour distribuer le conjugué et la solution de révélation. La réaction enzymatique est très sensible à tous métaux ou ions métalliques. Par conséquent, aucun élément métallique ne doit entrer en contact avec les différentes solutions contenant le conjugué ou le chromogène. Le Chromogène (R9) doit être incolore. L apparition d une coloration bleue indique que le réactif est inutilisable et doit être remplacé. Utiliser un cône de distribution neuf pour chaque échantillon. Vérifier l exactitude des pipettes et le bon fonctionnement des appareils utilisés. CONSIGNES D HYGIENE ET DE SECURITE Les composants d origine humaine utilisés dans la préparation des réactifs ont été testés et trouvés non réactifs pour l antigène de surface de l hépatite B (Ag HBs), les anticorps dirigés contre le virus de l hépatite C (anti-vhc) et les anticorps dirigés contre les virus de l immunodéficience humaine (anti-vih1 et anti-vih2). Du fait qu aucune méthode ne peut garantir de façon absolue l'absence d agents infectieux, considérer les réactifs d origine humaine ainsi que tous les échantillons de patients comme potentiellement infectieux et les manipuler avec les précautions d'usage : Considérer le matériel directement en contact avec les échantillons et les réactifs d origine humaine ainsi que les solutions de lavage comme des produits contaminés. Porter des gants à usage unique lors de la manipulation des réactifs et des échantillons. Ne pas pipeter à la bouche. Eviter les éclaboussures d échantillons ou de solutions les contenant. Nettoyer les surfaces souillées avec de l eau de javel diluée à 10 %. Si le liquide contaminant est un acide, neutraliser au préalable les surfaces souillées avec du bicarbonate de soude, puis nettoyer à l aide d eau de javel et sécher avec du papier absorbant. Le matériel utilisé pour le nettoyage devra être jeté dans un conteneur spécial pour déchets contaminés. Eliminer les échantillons, les réactifs d origine humaine ainsi que le matériel et les produits contaminés après décontamination : 22

23 - soit par immersion dans de l eau de javel à la concentration finale de 5 % d hypochlorite de sodium pendant 30 minutes. - soit par autoclavage à 121 C pendant 2 heures minimum. ATTENTION : ne pas introduire dans l autoclave des solutions contenant de l hypochlorite de sodium Eviter tout contact des réactifs, y compris ceux considérés comme non dangereux, avec la peau et les muqueuses. La manipulation et l élimination des déchets chimiques et biologiques doivent être faites selon les Bonnes Pratiques de Laboratoire. Tous les réactifs de la trousse sont destinés au seul usage diagnostic in vitro. Attention : certains réactifs contiennent du ProClin 300 < 1,5% R43 : Peut entraîner une sensibilisation par contact avec la peau S28-37 : Après contact avec la peau, se laver immédiatement et Xi - Irritant abondamment avec de l eau et du savon. Porter des gants appropriés 6. ECHANTILLONS 1. Les tests sont effectués sur des échantillons de sérum ou de plasma recueilli sur anticoagulant de type EDTA, héparine ou citrate. 2. Respecter les consignes suivantes pour le prélèvement, le traitement et la conservation de ces échantillons de sang : Prélever un échantillon de sang selon les pratiques en usage. Pour les sérums, laisser le caillot se former complètement avant centrifugation. Conserver les tubes fermés. Après centrifugation, extraire le sérum ou le plasma et le conserver en tube fermé. Les échantillons seront conservés à +2-8 C si le test est effectué dans les 7 jours. Si le test n est pas effectué dans les 7 jours, ou pour tout envoi, les échantillons seront congelés à -20 C (ou plus froid). Il est recommandé de ne pas procéder à plus de cinq cycles de congélation / décongélation. Les échantillons devront être soigneusement homogénéisés (Vortex) après décongélation et avant la réalisation du test. 3. Les résultats ne sont pas affectés par les échantillons contenant 90 g/l d albumine ou 100 mg/l de bilirubine non conjuguée, les échantillons lipémiques contenant l équivalent de 36 g/l de trioléïne (triglycéride) ou les échantillons hémolysés contenant 10 g/l d hémoglobine. 4. 24 7. MODE OPÉRATOIRE 7.1 Matériel nécessaire non fourni Agitateur type Vortex. Appareil de lecture pour microplaques équipé de filtres 450/620 nm (*). Incubateur de microplaques pouvant être thermostaté à 37 C ± 1 C (*). Système de lavage automatique, semi-automatique ou manuel pour microplaques (*). Eau distillée ou désionisée stérile. Gants à usage unique. Lunettes de protection. Papier absorbant. Pipettes ou multipipettes, automatiques ou semi- automatiques, réglables ou fixes, pouvant mesurer et délivrer 10 µl à 1000 µl, 1 ml, 2 ml et 10 ml. Eprouvettes graduées de 25 ml, 50 ml, 100 ml et 1000 ml. Hypochlorite de sodium (eau de javel) et bicarbonate de sodium. Conteneur de déchets contaminés. Tubes à usage unique (*) Nous consulter pour une information précise concernant les appareils validés par nos services techniques. 7.2 Reconstitution des réactifs R1: Laisser revenir 30 minutes à température ambiante ( C) avant ouverture du sachet. Sortir le cadre et replacer immédiatement les barrettes non utilisées dans le sachet en vérifiant la présence du dessicant. Refermer soigneusement le sachet et le replacer à +2-8 C. R2: Diluer au 1/20 la solution R2 avec de l eau distillée : 50 ml de R2 dans 950 ml d eau distillée. On obtient ainsi la solution prête à l emploi. Prévoir 350 ml de solution de lavage diluée pour une plaque entière de 12 barrettes en lavage manuel. R3, R4a, R4b, R4c, R4d: Diluer au 1/21 avec le Diluant (R7) (exemple : 300 µl de R µl de Calibrateur). R6+R7: Le Conjugué R6 est présenté sous forme liquide concentré 51 fois. Homogénéiser avant utilisation. Diluer au 1/51 avec le Diluant (R7). Pour une plaque complète, diluer extemporanément 0,5 ml de Conjugué (R6) dans 25 ml de Diluant (R7). Diviser les volumes par 10 pour une barrette. 7.3 Conservation et validité des réactifs ouverts et/ou reconstitués La trousse doit être conservée à +2-8 C. Chaque élément de la trousse conservée avant ouverture à +2-8 C peut être utilisé jusqu à la date de péremption indiquée sur le coffret. 24

25 R1: Après ouverture, les barrettes conservées dans le sachet correctement refermé sont stables pendant 8 semaines à +2-8 C (vérifier la présence du dessicant). R2: Après dilution, la Solution de Lavage se conserve 2 semaines à C. Après ouverture et en l absence de contamination, la Solution de Lavage concentrée peut être conservée à C jusqu à la date indiquée sur l étiquette. R3, R4a, R4b, R4c, R4d, R6, R7: Après ouverture et en l absence de contamination, les réactifs conservés à +2-8 C sont stables pendant 8 semaines. R6+R7: Après dilution, la solution de travail du conjugué est stable 8 heures à température ambiante ( C) ou 24 heures à +2-8 C. R9: Après ouverture et en l absence de contamination, le réactif conservé à +2-8 C est stable pendant 8 semaines. R10: Après ouverture et en l absence de contamination, le réactif conservé à +2-8 C est stable jusqu à la date d expiration indiquée sur l étiquette. 7.4 Mode opératoire Suivre strictement le protocole proposé et appliquer les Bonnes Pratiques de Laboratoire. Avant utilisation, laisser tous les réactifs revenir à température ambiante ( C). L utilisation de puits sécables requiert une attention particulière lors de la manipulation. Utiliser tous les calibrateurs à chaque mise en œuvre du dosage pour valider la qualité du test. 1. Etablir soigneusement le plan de distribution et d identification des calibrateurs et des échantillons de patients. 2. Préparer la Solution de Lavage diluée (R2) [Se référer au chapitre 7.2]. 3. Sortir le cadre support et les barrettes (R1) de l emballage protecteur [Se référer au chapitre 7.2]. 4. Dans des tubes identifiés individuellement, diluer les calibrateurs R3, R4a, R4b, R4c, R4d et les échantillons de patient à tester au 1/21 dans le Diluant (R7), soit 300 µl de Diluant (R7) puis 15 µl d échantillon [Se référer au chapitre 7.2]. Bien homogénéiser (Vortex). 5. Distribuer dans chaque cupule 200µl des calibrateurs et des échantillons dilués selon le schéma suivant : 25

Puis incuber immédiatement la microplaque au bain-marie thermostaté ou dans un incubateur sec de microplaques pendant 1 heure ± 5 minutes à 37 C ± 1 C. 7. Avant la fin de la première incubation, préparer la solution de travail du conjugué (R6+R7) [Se référer au chapitre 7.2]. 8. A la fin de la première incubation, retirer le film adhésif, aspirer le contenu de toutes les cupules dans un conteneur pour déchets contaminés (contenant de l hypochlorite de sodium) et procéder à 4 lavages avec 350 µl de la Solution de Lavage (R2). Sécher les barrettes par retournement sur une feuille de papier absorbant et taper légèrement afin d éliminer la totalité de la Solution de Lavage. 9. Distribuer immédiatement 200 µl de la solution de travail du conjugué (R6+R7) dans toutes les cupules. Agiter délicatement cette solution avant l emploi. 10. Couvrir la microplaque d un film adhésif neuf en appuyant bien sur toute la surface pour assurer l étanchéité. Incuber la microplaque au bain-marie thermostaté ou dans un incubateur sec de microplaques pendant 1 heure ± 5 minutes à 37 C ± 1 C. 11. A la fin de la deuxième incubation, retirer le film adhésif, aspirer le contenu de toutes les cupules dans un conteneur pour déchets contaminés (contenant de l hypochlorite de sodium) et procéder à 4 lavages avec 350 µl de la Solution de Lavage (R2). Sécher les barrettes par retournement sur une feuille de papier absorbant et taper légèrement afin d éliminer la totalité de la Solution de Lavage. 12. Distribuer rapidement, et à l'abri de la lumière vive, 200 µl du Chromogène (R9) dans toutes les cupules. Laisser la réaction se développer à l obscurité pendant 30 ± 5 minutes à température ambiante ( C). Lors de cette incubation, ne pas utiliser de film adhésif. 26

27 13. Arrêter la réaction enzymatique en ajoutant 100 µl de la Solution d Arrêt (R10) dans chaque cupule. Adopter la même séquence et le même rythme de distribution que pour la solution de révélation. 14. Essuyer soigneusement le dessous des plaques. Lire la densité optique à 450/620 nm à l aide d un lecteur de plaques dans les 30 minutes qui suivent l arrêt de la réaction. Les barrettes doivent toujours être conservées à l abri de la lumière avant la lecture. 15. S'assurer, avant la transcription des résultats, de la concordance entre la lecture et le plan de distribution des plaques et des échantillons. 8. INTERPRÉTATION DES RÉSULTATS 8.1 Etablissement de la courbe d étalonnage Aucun standard international n étant disponible pour le dosage des IgG anti- CMV, le réactif Platelia CMV IgG est calibré par rapport à la préparation standard proposée par l OMS en La présence et la quantité d anticorps de classe IgG dirigés contre le CMV dans l échantillon testé sont déterminées en comparant la densité optique (DO) de l échantillon aux valeurs des calibrateurs de la gamme d étalonnage exprimées en Unités Arbitraires par millilitre (UA/ml). Tracer la courbe d étalonnage [DO = fonction (UA/ml)] en reportant sur l axe vertical (axe des Y) les DO des calibrateurs R3, R4a, R4b, R4c et R4d, puis en reportant sur l axe horizontal (axe des X) leur concentration respective en UA/ml. Pour chaque échantillon testé, calculer le titre en anticorps IgG anti- CMV en déterminant à partir de la courbe d étalonnage ainsi tracée la concentration correspondante à la DO mesurée. 8.2 Validation de l'essai Analyser les résultats de DO obtenus avec les calibrateurs sur chaque microplaque et pour chaque série. Pour valider la manipulation, les critères suivants doivent être respectés : Valeurs des densités optiques : DO R4a 0,150 DO R4b 0,300 DO R4d > DO R4c Rapports des densités optiques : DO R4a / DO R3 3,50 DO R4b / DO R4a 1,50 DO R4c / DO R4b 1,50 Si ces critères ne sont pas respectés, recommencer la manipulation. 27

Si une contamination du patient est suspectée, un second prélèvement doit être analysé environ deux semaines plus tard. Titre 0,5 UA/ml Positif Un résultat positif est le plus souvent le témoin d une infection ancienne. Cependant, une infection récente ne peut être exclue, notamment en présence d anticorps IgM anti-cmv. En cas de suspicion d infection récente, des tests sérologiques complémentaires tels que la mesure de l avidité des IgG peuvent être utiles pour confirmer le diagnostic. Une plus forte variation des titres peut être observée au delà d une concentration de 4 UA/ml. Les échantillons de concentration supérieure au dernier point de la gamme de calibration peuvent être dilués dans le diluant R7 afin d obtenir un résultat situé dans la zone de linéarité du dosage. Dans ce cas, la concentration finale est obtenue en multipliant le résultat trouvé par le taux de dilution utilisé. 8.4 Expertise des causes d erreur L origine des réactions non validées ou non reproductibles est souvent en relation avec les causes suivantes : Lavage insuffisant des microplaques. Contamination des échantillons négatifs par un sérum ou un plasma contenant un titre élevé d anticorps. Contamination ponctuelle de la solution de révélation par des agents chimiques oxydants (eau de javel, ions métalliques...). Contamination ponctuelle de la Solution d Arrêt. 9. PERFORMANCES Les performances du kit Platelia CMV IgG ont été évaluées sur 2 sites sur un total de 1096 échantillons provenant de femmes enceintes et de donneurs de sang. 28

29 9.1 Prévalence La prévalence des IgG anti-cmv mesurée à l aide du test Platelia CMV IgG a été estimée sur un panel de 300 échantillons provenant de femmes enceintes issues de la région parisienne (France). 145 échantillons se sont révélés positifs pour la présence d anticorps anti-cmv IgG.
