Objectivos: Comparar a função miocárdica regional por ecocardiografia Doppler tecidular (DT), entre doentes com miocardiopatia hipertrófica obstrutiva (MHO) e doentes com forma não obstrutiva (MHNO) da doença. Metodologia: Em 26 doentes com MHO e em 23 com MHNO foi analisada por DT a função miocárdica regional ventricular esquerda no eixo longitudinal (8 segmentos, via apical) e radial (2 segmentos, via paraesternal, curto eixo). Foram analisados parâmetros no domínio das velocidades, tempos e integral velocidade-tempo, bem como os índices de heterogeneidade e assincronia, e em cada parede, o gradiente meridional (basal-mediano) de velocidades. Estes dados foram comparados em cada grupo e entre os grupos. Resultados: O grupo MHO apresentou, em relação ao grupo MHNO: Função sistólica: a) longitudinal: velocidades, intervalos temporais e integrais semelhantes; b) radial: maior gradiente meridional, menores integrais velocidade tempo. Função diastólica: a) longitudinal: menor a, ABSTRACT Regional Myocardial Function by Tissue Doppler in Hypertrophic Cardiomyopathy: The Impact of Obstruction Background: The conventional echocardiographic assessment of myocardial function in patients with obstructive hypertrophic cardiomyopathy (HOCM) is complex, because of the load dependency of this method. Tissue Doppler imaging (TDI) may improve this evaluation. Aim: To compare regional myocardial function with TDI, between patients with hypertrophic obstructive cardiomyopathy (HOCM) and with non-obstructive forms of the disease (NOHCM). Methods: 26 patients with HOCM and 23 with NOHCM were studied with pulsed TDI. We studied longitudinal (8 left ventricular segments, apical views) and radial regional function (2 segments, short axis view), and analyzed velocities, time intervals, velocitytime integrals and heterogeneity and asynchrony indices and the meridional (basal-medial segments) velocity gradient in each wall. Data were compared within each group and between groups. Results: Compared to NOHCM, HOCM patients showed: systolic functions: a) longitudinal: similar velocities, time intervals and integrals; b) radial: higher meridional gradient, lower velocity-time integrals. Conclusões: Este trabalho revela que em doentes com MH, a presença de obstrução intraventricular tem diferente impacto na função miocárdica regional sistólica e diastólica, longitudinal e radial, avaliada por DT. Assim, nos doentes com MHO: 1-A função sistólica regional no eixo longitudinal é essencialmente semelhante à das formas não obstrutivas, traduzindo relativa independência desta em relação à carga. 2-A função diastólica regional, longitudinal e radial, difere significativamente das formas não obstrutivas, o que pode reflectir a influência das condições de carga na função diastólica regional. Conclusions: This study shows that in HOCM patients, the presence of obstruction and its associated load conditions have a different impact on systolic and diastolic regional myocardial function, in long and short axis, assessed with TDI. So, in HOCM patients: 1 - Long axis regional systolic function is similar to the non-obstructive forms, suggesting relative load independence. 2 - Long and short axis regional diastolic function is, in specific segments and parameters, different from the nonobstructive forms. These data should be taken into account in the assessment of regional myocardial function with TDI in HOCM. Key words Regional longitudinal and radial myocardial function; Tissue Doppler echocardiography; Obstructive and non-obstructive hypertrophic cardiomyopathy 1414 INTRODUÇÃO Amiocardiopatia hipertrófica (MH) é uma doença primária do músculo cardíaco, que se caracteriza por hipertrofia miocárdica independente das condições de carga. Dado que nesta entidade o defeito primário consiste na produção de proteínas sarcoméricas anormais, esta hipertrofia é, no entanto, um mecanismo compensatório ineficaz, tentando suprir quantitativamente o defeito funcional miocitário (1-3). Assim, apesar da maior massa miocárdica, estes doentes apresentam alterações da contractilidade, relaxamento e distensibilidade ventricular (1-3). Em cerca de 1 /3 dos casos, existe obstrução intraventricular em repouso, designando-se então a doença por miocardiopatia hipertrófica obstrutiva. Do ponto de vista fisiopatológico, a obstrução representa aumento da pós carga; por outro lado, como esta situação se associa a disfunção diastólica e insuficiência mitral, a précarga também se encontra aumentada (1-3). Apesar de não estar provado que a obstrução interfira no prognóstico, a sua presença influencia a sintomatologia e a sua redução ou INTRODUCTION Hypertrophic cardiomyopathy (HCM) is a primary disease of cardiac muscle characterized by myocardial hypertrophy independent of load conditions. As the primary defect in this condition is the production of abnormal sarcomeric proteins, this hypertrophy is an ineffective compensatory mechanism that attempts to suppress quantitatively a functional defect of the myocytes (1-3). Thus, despite their greater myocardial mass, these patients present abnormalities in contractility, relaxation and ventricular distensibility (1-3). In around a third of cases, there is intraventricular obstruction at rest, and the disease is then termed obstructive hypertrophic cardiomyopathy. From a pathophysiological standpoint, obstruction represents an increase in afterload, but at the same time preload is also increased (1-3), as there is associated diastolic dysfunction and mitral regurgitation. Although it has not been proved that obstruction affects prognosis, its presence influences symptomatology and reducing or eliminating it improves symptoms and effort tolerance (1-3).

A Ecocardiografia Doppler tecidular (15-17) é uma técnica ultrasónica que visa complementar o estudo convencional, podendo ultrapassar as limitações acima referidas, quantificando a função regional com menor dependência das condições de carga (18-20). Estudos prévios com este método em doentes com MH hipertrófica revelam disfunção sistólica (21, 22) e diastólica (22-25) regional, predominantemente longitudinal (26), em paredes com e sem hipertrofia (27). Myocardial function in HCM is generally (4, 5) assessed by M-mode, two-dimensional and Doppler (7-10) echocardiography, which provide invaluable information for its assessment. However, some of the classic limitations of these techniques, such as their dependency on load conditions and their inability to quantify regional function, become more significant in a complex disease such as HCM. The results are often difficult to interpret, with poor correlation with hemodynamic data, particularly in the obstructive forms (12-14). Tissue Doppler echocardiography (15-17) is an ultrasound technique that complements the conventional Doppler study, enabling some of the limitations described above to be overcome, by quantifying regional function with less dependency on load conditions (18-20). Previous studies using this method in patients with HCM have shown regional systolic (21, 22) and diastolic (22-25) dysfunction, predominantly longitudinal (26), in walls with and without hypertrophy (27). Critérios de exclusão: diabetes mellitus, causa conhecida potencialmente responsável pela hipertrofia, nomeadamente HTA ou valvulopatias; classe funcional 3 e 4 NYHA; frequência cardíaca >90/minuto, ausência de ritmo sinusal, intervalo PR <120 ms ou >200 ms, bloqueio completo de ramo esquerdo ou direito; pacemaker, má janela ecocardiográfica, regurgitação mitral grau 3 ou 4, evidência de doença coronária epicárdica (perante suspeita de doença coronária os doentes eram submetidos a coronariografia, tendo sido POPULATION The patients with HCM included in this study were those that fulfilled the inclusion and exclusion criteria. Inclusion criteria: age over 45; myocardial hypertrophy; segmental myocardial thickness of over 15 mm, with an interventricular septum/posterior wall ratio (or anterior wall/inferior wall ratio) greater than 1.3, or apical form. Exclusion criteria: diabetes mellitus; a known potential cause of hypertrophy, particularly hypertension or valve disease; NYHA functional class III or IV; heart rate >90 bpm; absence of sinus rhythm, PR interval <120 ms or >200 ms; complete left or right bundle branch block; pacemaker; poor echocardiographic window; grade 3 or 4 mitral regurgitation; evidence of epicardial coronary disease (all patients with suspected coronary disease underwent coronary angiography, and any with epicardial coronary disease were excluded). Therapy with beta-blockers and/or calcium channel blockers was discontinued for at least 1415

Doentes medicados com amiodarona não suspenderam este fármaco. De acordo com a presença ou ausência de obstrução intraventricular em repouso os doentes foram divididos em dois grupos: formas não obstrutivas (MHNO, gradiente inferior a 30 mmhg) e formas obstrutivas (MHO, gradiente superior a 30 mmhg). O protocolo de estudo foi aprovado pela Comissão de Ética do nosso Hospital, tendo cada doente e voluntário fornecido consentimento informado. MÉTODOS Ecocardiografia convencional. Todos os exames foram efectuados em decúbito lateral esquerdo utilizando um ecocardiógrafo Toshiba SSA 380A Powervision (Toshiba Corp, Tóquio, Japão) e sonda de 3,75 mhz, phased array com 64 elementos, imagem fundamental, frame rate em bidimensional superior a 100/s usando as vias ecocardiográficas convencionais. A velocidade de varrimento usada nas medições foi de 100 mm/s, com gravação em vídeo para revisão posterior. De acordo com as recomendações da Sociedade Americana de Ecocardiografia, foram medidos em modo M, bidimensional (28) e Doppler 29 contínuo e pulsado os parâmetros habitualmente avaliados com este método. Como a exactidão dos métodos clássicos de quantificação de hipertrofia é menor na presença de hipertrofias assimétricas, foi igualmente calculado para cada doente um índice de hipertrofia ventricular esquerda (27), bem como os scores específicos habitualmente usados para avaliar a extensão e distribuição da hipertrofia na MH, nomeadamente a classificação morfológica de Maron (4), o score de Wigle (5) e o score de Spirito Maron (6). Como índices temporais de função sistólica global, mediram-se, a partir do fluxo de ejecção aórtico, os intervalos de tempo sistólicos globais, e calculou-se a relação PEP/LVET (pre ejection period período pré-ejecção/left ventricular ejection time tempo de ejecção) (30-33). A presença ou ausência de disfunção diastólica foi feita com base nas recomendações do grupo de estudos de insuficiência cardíaca dias- 48 hours before the exam. All the examinations were carried out in left lateral decubitus using a Toshiba SSA 380A Powervision echocardiograph (Toshiba Corp., Tokyo, Japan) with a 3.75 MHz probe, 64-element phased array, fundamental image, with a two-dimensional frame rate of over 100/s, using conventional echocardiographic views. The scanning speed used in the measurements was 100 mm/s, with video recording for subsequent analysis. The parameters usually assessed by this method were measured in M-mode, two-dimensional (28) continuous and pulsed Doppler (29) in accordance with the recommendations of the American Society of Echocardiography. Since conventional methods of quantifying hypertrophy are less accurate in the presence of asymmetric hypertrophy, a left ventricular hypertrophy index (27) was also calculated for each patient, as well as the specific scores generally used to assess the extent and distribution of hypertrophy in HCM, namely the Maron classification (4), the Wigle score (5) and the Spirito Maron score (6). As temporal indexes of global systolic function, the global systolic time intervals were measured on the basis of aortic ejection flow, and the PEP/LVET (pre-ejection period/left ventricular ejection time) ratio was also calculated (30-33). The presence or absence of diastolic dysfunction was established on the basis of the recommendations of the Study Group on Diastolic Heart Failure of the European Society of Cardiology (34) and the Canadian Consensus on echocardiography (35). In accordance with these criteria, if the patient presented with criteria of diastolic dysfunction, this was classified as delayed isovolumic relaxation or abnormal rela-
5 tólica da Sociedade Europeia de Cardiologia (34) e do Consenso Canadiano de ecocardiografia (35). De acordo com estes critérios, se o doente apresentasse critérios de disfunção diastólica, esta era classificada em padrão de atraso de relaxamento isovolumétrico ou relaxamento anormal (disfunção diastólica grau I), padrão de pseudo normalização (disfunção diastólica grau II), ou padrão restritivo (disfunção diastólica grau III ou IV), recorrendo à análise do fluxo venoso pulmonar (36, 37). Foi também avaliada a presença ou ausência de semi-encerramento aórtico e de SAM (movimento sistólico anterior da mitral). Em relação ao SAM, foi efectuada a sua caracterização temporal, em termos de início e duração de contacto com o septo. A regurgitação mitral foi classificada em ligeira, moderada ou grave, de acordo com a área planimetrada por Doppler codificado a cor (ligeira: área planimetrada <3 cm 2, moderada: >3 e <6 cm 2, grave >6 cm 2 ). Foi igualmente avaliada a presença de calcificação do anel mitral, anomalias primárias da válvula mitral e dos músculos papilares. Foi também pesquisada a presença de gradientes mesoventriculares. Ecocardiografia Doppler tecidular. Com este método, o Doppler pulsado convencional é modificado para registar os sinais de alta amplitude e baixa velocidade provenientes dos tecidos cardíacos. Uma das modalidades desta técnica é o Doppler tecidular pulsado, de elevada resolução temporal e velocimétrica, na qual uma amostra é colocada na região de interesse, sendo o perfil Doppler espectral velocidade-tempo referente à zona interrogada apresentado no écran. Em cada uma das ondas observadas, é possível efectuar medições de parâmetros do domínio de velocidade e de tempo, que caracterizam a função miocárdica regional sistólica e diastólica da área em estudo. Os ganhos e filtros foram optimizados para obter a melhor relação sinal-ruído, com limite de Nyquist de 20 cm/s. Em cada segmento analisado foram estudados 5 ciclos consecutivos, no final da expiração, tendo sido calculada a respectiva média. Na avaliação da função miocárdica regional (FMR) longitudinal, a amostra de volume, com 5 mm, foi colocada na região subendocárdica de 8 segmentos de quatro paredes do ventrículo esquerdo: segmento basal e mediano do septo interventricular (SIV), parede lateral (PL) xation pattern (grade I diastolic dysfunction), pseudonormalization pattern (grade II diastolic dysfunction), or restrictive pattern (grade III or IV diastolic dysfunction), using the early analysis of the pulmonary venous flow (36, 37). The presence or absence of early aortic closure and of mitral systolic anterior motion (SAM) were also determined. SAM was characterized temporally, in terms of beginning and duration of contact with the septum. Mitral regurgitation was classified as mild, moderate or severe, depending on the planimetric area by color Doppler (mild: planimetric area <3 cm 2, moderate: >3 and <6 cm 2, severe: >6 cm 2 ). The presence of calcification of the mitral annulus, primary abnormalities of the mitral valve and the papillary muscles, and midventricular gradients, were also determined. Tissue Doppler echocardiography. In this method, conventional pulsed Doppler imaging is modified to record high-amplitude, lowvelocity signals from cardiac tissue. One of the modalities of this technique is pulsed tissue Doppler imaging, which gives high time and velocity resolution. The sample is placed in the region of interest, and the spectral velocity-time Doppler profile of the zone interrogated is shown on the screen. Parameters in the velocity and time domains can be measured for each of the waves recorded, characterizing the regional systolic and diastolic myocardial function in the area under study. The gains and filters were optimized to obtain the best possible signal-to-noise ratio, with a Nyquist limit of 20 cm/s. In each segment examined, 5 consecutive cycles were studied at end-expiration and the respective mean was calculated. In the assessment of longitudinal regional myocardial function (RMF), the 5-mm sample volume was positioned in the subendocardial region of the eight segments of the four walls of the left ventricle: basal and medial segments of the intraventricular septum (IVS), lateral wall (LW) (4-chamber apical view), inferior wall (IW) and anterior wall (AW) (2-chamber apical view). In the analysis of radial RMF, the 5 mm sample volume, was positioned in the intramyocardial region of the basal and medial segments of the inferior wall in short axis left parasternal view. Myocardial velocities. In each of the segments analyzed, maximum velocities (cm/s) were measured of the systolic (s), rapid relaxa- 1417

Na análise da FMR radial, a amostra de volume, também com 5 mm, foi colocada na região intramiocárdica do segmento basal e mediano da parede inferior em plano para-esternal esquerdo, curto eixo. Velocidades miocárdicas. Em cada um dos segmentos analisados mediram-se as velocidades máximas (cm/s) das ondas sistólica (s), de relaxamento rápido ou diastólica precoce (e) e de relaxamento tardio (a), tendo-se calculado a relação e/a. Para cada parâmetro, no eixo longitudinal, foi calculado o respectivo índice de heterogeneidade, que representa a variação fisiológica das velocidades nos oito segmentos ventriculares analisados (índice de heterogeneidade HI= x-m /8, em que x é a velocidade máxima da onda em cada um dos segmentos estudados e m é a média das velocidades nesses locais. Foi ainda calculado, em cada grupo, a percentagem de segmentos longitudinais em que a relação e/a é superior a 1, bem como o número médio de segmentos com e/a maior que 1 por indivíduo. Foi também calculado, no eixo longitudinal e radial, o gradiente meridional, que consiste na diferença de velocidades (não corrigido para unidade de espaço) entre os segmentos basais e medianos do VE. Intervalos temporais. A duração de cada onda (ms) foi, quando necessário, extrapolada prolongando a respectiva pendente de aceleração/desaceleração até à linha de base. O registo foi feito com monitorização electro e fonocardiográfica simultânea, tendo-se medido a partir da onda R do ECG (S1 do fono) 9 intervalos de tempo (t1 a t9): t1-de R (S1 no fono) até início de S «tempo de contracção isovolumétrica regional» («TCI») ou tempo pré-contração = PEP «pre ejection period»; t2 - até pico de s; t3 - até fim de s-tempo de sístole; t4 - até início de e; t5 - até pico de e; t6 - até fim de e; t7 - até início de a; t8 - até pico de a; t9 - até fim de a. A partir desses intervalos foram calculados outros intervalos de tempo: tempo de ejecção ou de contração = t3-t1 = LVET «left ventricular ejection time»; PEP/LVET = t1/t3-t1; «TRI» («tempo de relaxamento isovolumétrico regional») ou tempo pré relaxamento = t4-t3; Tempo até pico de e = A2-e = t5-t3; Tempo de diástole = t9-t3. Através do coeficiente de variação dos vários intervalos temporais nos diferentes segmentos, no plano longitudinal, foram calculados para cada intervalo de tempo os indíces de assincronia sistólica diastólica, medidas de heterogeneidade dos intervalos de tempo. tion or early diastolic (e) and late relaxation (a) waves, and the e/a ratio was calculated. For each parameter, in long axis view, the respective heterogeneity index was calculated, which represents the physiological variation of the velocities in the eight ventricular segments analyzed (heterogeneity index HI= x-m /8, where x is the maximum velocity of the wave at each of the segments studied and m is the mean of the velocities at these sites). The number of longitudinal segments in which the e/a ratio is greater than 1 was calculated for each group, as was the mean number of sites with e/a equal to or greater than 1 per individual. The meridional gradient, being the difference in velocities (not corrected for the unit of space) between the basal and medial segments of the left ventricle, was also calculated in the long and short axis. Time intervals. The duration of each wave, in ms, was extrapolated, when necessary, by extending the respective curve of acceleration/deceleration to the baseline. The measurements were made using simultaneous electrocardiography and phonocardiography, with 9 time intervals (t1 to t9) being measured from the R wave of the ECG (S1 phonocardiographic): t1 - from R (S1 phonocardiographic) up to beginning of S - regional isovolumic contraction time (ICT) or pre-contraction time = pre-ejection period (PEP); t2 - up to peak of s; t3 - up to end of s - time of systole; t4 - up to beginning of e; t5 - up to peak of e; t6 - up to end of e; t7 - up to beginning of a; t8 - up to peak of a; t9 - up to end of a. On the basis of these intervals other time intervals were calculated: ejection time or contraction time= t3-t1=lvet, left ventricular ejection time; PEP/LVET=t1/t3-t1; IRT ( regional isovolumic relaxation time ) or pre-relaxation time=t4-t3; Time to peak of e=a2-e=t5- t3; Time of diastole=t9-t3. For each time interval, the systolic and diastolic asynchrony indexes, which are measures of the heterogeneity of the time intervals, were calculated, on the basis of the coefficient of variation of the various time intervals in the different segments in long axis view. Velocity-time integrals. The velocitytime integral for each wave was measured, which is a measure of amplitude of movement, expressed in mm. Statistical analysis. The results are presented as means and standard deviations. The Student s t test and the chi-square test were

Os resultados são apresentados como média e desvio padrão. O teste t de Student e o teste do qui-quadrado foram usados para comparar os dados clínicos e ecocardiográficos nos 2 grupos. Para investigar a função miocárdica longitudinal, compararam- -se os valores de cada variável entre os quatro segmentos ventriculares basais e entre os quatro segmentos medianos nos mesmos indivíduos, utilizando-se a análise de variância a dois factores. Nos casos em que a análise mostrava a existência de uma diferença entre localizações, para um nível de significância de 5 %, fez-se uma análise post-hoc das diferenças entre cada duas localizações pelo teste de Student para amostras emparelhadas com correcção de Bonferroni. Em seguida, investigou-se a hipótese de não existirem diferenças entre os grupos no conjunto das medições nas quatro localizações de cada estrutura cardíaca em cada indivíduo, por análise de variância a dois factores com medições repetidas num factor. O modelo ajustado foi o de uma análise de variância de efeitos mistos a três factores, representando os grupos de indivíduos e as localizações os factores fixos e os indivíduos o factor aleatório. Nesta análise testou-se simultaneamente a hipótese de os grupos não diferirem relativamente aos valores médios através das diferentes localizações das medições, e a hipótese das diferenças entre grupos não serem constantes em todas as quatro localizações. Este última hipótese foi testada pela inclusão no modelo de um termo para a interacção entre os grupos e as localizações. Por questões de potência, aceitou-se como evidência de uma diferença um teste significativo para um nível bilateral de 10 %. Em todos os outros casos, as diferenças foram testadas para um nível de significância de 5 % por testes bilaterais. A reprodutibilidade das medições foi testada em 10 doentes consecutivos, 5 de cada grupo, através de regressão linear e método de Bland e Altman (38). Os cálculos foram efectuados com o programa STATA release 7.0 (Stata Corporation, Colleage Station, TX, USA). used to compare the clinical and echocardiographic data in the two groups. To analyze longitudinal myocardial function, the values of each variable were compared between the four basal ventricular segments and between the four medial segments in the same individuals, using two-way analysis of variance. In cases where the analysis revealed a difference between segments, to a level of significance of 5 %, a post-hoc analysis was carried out of the differences between each two segments using the Student s t test for paired samples with Bonferroni s correction. Next, the hypothesis was tested that there were no differences between the groups in the series of measurements at the eight segments in each individual, using two-way analysis of variance with repeated measures in one factor. The model adopted was that of a three-way analysis of variance with mixed effects, with the groups of individuals and the segments the fixed factors and the individuals the random factor. This analysis simultaneously tested the hypothesis that the groups did not differ with respect to the mean values obtained at the different sites of the measurements, and the hypothesis that the differences between the groups were not constant in all eight segments. The latter hypothesis was tested by including in the model a term for the interaction between the groups and the segments. For reasons of power, a test significant to a bilateral level of 10% was accepted as evidence of a difference. In all other cases, the differences were tested to a level of significance of 5 % by bilateral tests. The reproducibility of the measurements was tested in 10 consecutive individuals, 5 from each group, using linear regression and Bland and Altman s method (38). The calculations were performed using the STATA program, release 7.0 (Stata Corporation, College Station, Texas, USA). RESULTS The study population consisted of 26 patients with HOCM, 5 male and 21 female, aged between 51 and 78 (mean 66±7), and 23 patients with NOHCM, 13 male and 10 female, aged between 46 and 81 (mean 61±10). Wall thickness was similar in the two groups at the septum and anterior wall, but the non-obstructive group had thinner posterior, inferior and lateral walls. The distribution according to the Maron classification was as follows: HOCM group: type 1=4 (15 %), type 2=7 (27 %), type 3=15 (58 %), type 4 (apical)=0 (0 %). The mean Wigle score in this population was 4.9±1.2. NOHCM group: type 1=5 (22 %), type 2=7 (30 %), type 3=9 (39 %), type 4=2 (9 %); Wigle score 3.7±1.2. With regard to systolic function, fractional shortening was similar in the two groups, but for systolic time intervals the HOCM group showed shorter ICT and longer ejection time than the non-obstructive group, leading to a much lower PEP/LVET ratio.

Grupo MHNO: tipo 1= 5 (22 %), tipo 2= 7 (30 %), tipo 3= 9 (39 %), tipo 4= 2 (9 %); score de Wigle 3,7±1,2. No que diz respeito à função sistólica, a fracção de encurtamento foi semelhante nos 2 grupos, mas o grupo com MHO apresenta, no que diz respeito aos intervalos de tempo sistólicos, menor TCI e maior tempo de ejecção que o grupo não obstrutivo, condicionando PEP/ LVET muito inferior. Em relação à função diastólica global, o grupo MHO revelou maior velocidade da onda de enchimento rápido E e de contracção auricular A, com maior relação E/A e menor tempo de desaceleração. Dos 26 doentes com MHO 13 (50 %) apresentavam critérios de relaxamento anormal, 12 (46 %) padrão de pseudo-normalização e 1 (4 %) padrão restritivo, enquanto que nos 23 doentes com forma não obstrutiva da doença, 18 (78 %) tinham padrão de relaxamento anormal e 5 (22 %) de pseudo-normalização. No que diz respeito à regurgitação mitral, no grupo MHO, 17 doentes apresentavam insuficiência mitral ligeira e 9 moderada. Na população MHNO, 15 doentes não tinham regurgitação mitral ou esta era mínima (área <1 cm 2 ) e 16 apresentavam regurgitação ligeira, tendo apenas 2 regurgitação moderada. Não se verificaram diferenças entre os grupos quanto à presença de calcificação do anel mitral (MHNO 31 %; MHO 37 %), nem quanto à sua gravidade. Igualmente os grupos eram semelhantes quanto à presença de anomalias do aparelho valvular mitral (MHNO: 21 %; displasia n=3; músculo papilar anormal n=1, falsa corda n=1); (MHO 26 %: displasia n=3, músculo papilar anormal n=4). Apenas 1 doente com MHNO apresentava semi-encerramento da válvula aórtica, enquanto que este estava presente em todos os com MHO. Dos doentes com MHNO, só 21 % apresentavam SAM, que no entanto nunca contactava com o septo. Ao contrário, todos os doentes com MHO tinham SAM, holossistólico em 16 (61 %), iniciando-se no terço inicial da sístole em 20 (77 %). In terms of global diastolic function, the HOCM group presented higher velocity of the rapid filling wave E and of the atrial contraction wave A, with higher E/A ratio and shorter deceleration time. Of the 26 patients with HOCM, 13 (50 %) presented criteria of abnormal relaxation, 12 (46 %) pseudonormalization pattern and 1 (4 %) restrictive pattern, while of the 23 patients with the non-obstructive form of the disease, 18 (78 %) had abnormal relaxation pattern and 5 (22 %) pseudonormalization. With regard to mitral regurgitation, in the HOCM group 17 patients presented mild and 9 patients moderate mitral regurgitation. In the NOHCM population, 15 patients had no or minimal mitral regurgitation (area <1 cm 2 ) and 16 presented mild regurgitation, with only two having moderate regurgitation. No differences were found between the groups in terms of the presence of calcification of the mitral annulus (NOHCM 31 %; HOCM 37 %), or in its severity. The groups were also similar in terms of the presence of abnormalities of the mitral valve apparatus (NOHCM: 21 %, dysplasia n=3, abnormal papillary muscle n=1, false chordae tendineae n=1; HOCM: 26 %, dysplasia n=3, abnormal papillary muscle n=4). Only one patient with NOHCM presented early aortic valve closure, while this was found in all the HOCM patients. Of the patients with NOHCM only 21 % presented SAM, which however did not contact with the septum. In contrast, all the patients with OHCM had SAM, pansystolic in 16 (61 %), and beginning in the initial third of systole in 20 (77 %). Regional systolic function, myocardial velocities (Table II) Longitudinal function. In the NOHCM group, in the basal segments systolic velocities were lower at the anterior wall and higher at the inferior wall; no differences were seen between the various walls in medial segments, which explains the interaction seen (<0.01). In the HOCM group, no differences were found between the velocities at any of the walls. The meridional gradient was lower in both groups at the septum, where it was negative. No significant differences were found between the groups with regard to systolic velocities, meridional gradient or heterogeneity index (NOHCM: 1.23±0.43; HOCM: 1.1±0.44). DISCUSSÃO Múltiplos estudos, hemodinâmicos (39-42), cintigráficos (43,44) e ecocardiográficos (45-50), demonstram que as condições de carga ventriculares estão alteradas na MH. De acordo com estes estudos, e em concordância com a lei de Laplace, na MHNO a pós-carga encontra-se diminuída, estando a pré-carga aumentada, como consequência de disfunção diastólica e regurgitação mitral. Na MHO, como consequência da obstrução intraventricular, a pós-carga está aumentada (21), estando a pré-carga também aumentada devido à maior gravidade da insuficiência mitral e da disfunção diastólica. Tendo em conta estes aspectos, a avaliação correcta da função miocárdica global na MH por Doppler convencional, muito dependente da carga por analisar fluxos, torna-se extremamente complexa, sendo as correlações com a hemodinâmica deficientes (12, 13, 14). Assim, e de acordo com estes conceitos, os aparentemente melhores índices temporais de função sistólica do grupo MHO (menor PEP, maior LVET, menor PEP/LVET) e a elevada velocidade do fluxo de ejecção apenas confirmam a influência da obstrução nestes parâmetros, não traduzindo melhor função sistólica global (39-50). Do mesmo modo, os índices de função diastólica transmitral do grupo MHO (maior E/A, menor tempo de desaceleração) reflectem o efeito da pré-carga, e não traduzem melhor função diastólica (7-14), estando o maior número de doentes com pseudonormalização (inter-observer). DISCUSSION Many studies, hemodynamic (39-42), scintigraphic (43, 44) and echocardiographic (45-50), show that ventricular load conditions are altered in HCM. According to these studies, and following Laplace s law, afterload is reduced and preload is increased in NOHCM, as a consequence of diastolic dysfunction and mitral regurgitation. In HOCM, afterload is increased as a result of intraventricular obstruction (21), with preload also being increased due to the greater severity of mitral regurgitation and diastolic dysfunction. Given these facts, the accurate assessment of global myocardial function in HCM by conventional Doppler, which analyzes flows and is therefore highly load-dependent, becomes extremely complex, and correlations with hemodynamics are poor (12, 13, 14). Thus, according to this argument, the apparently better temporal indices of systolic function in the HOCM group (lower PEP, longer LVET, lower PEP/LVET) and the high velocity of ejection flow only confirm the influence of obstruction on these parameters, and do not reflect better global systolic function (39-50). In the same way, indices of transmitral diastolic function in the HOCM group (greater E/A, shorter deceleration time) reflect the effect of preload, and do not mean better diastolic function (7-14), with a larger number of patients with pseudonormalization of flow and with moderate mitral regurgitation being found in the obstructive group according to this reasoning. Since the analysis with TDI is performed at the myocardial level and not with flow, being less affected by load conditions, this technique can be useful in assessing patients with HOCM. However, given that a comparison between regional myocardial function in patients with HCM with and without obstruction had yet to be described, we decided to make such a comparison. Our results demonstrate that the presence of obstruction does not affect regional longitudinal systolic function, which is fundamentally the same as in the non-obstructive forms, showing that this function is independent of load

No entanto, dado que a comparação entre função miocárdica regional de doentes com MH com e sem obstrução ainda não foi descrita, decidimos efectuá-la. Os nossos resultados revelam que a presença de obstrução não afecta a função sistólica regional longitudinal, que é essencialmente semelhante à das formas não obstrutivas, traduzindo independência desta função em relação à carga. O facto de as diferenças entre os grupos terem ocorrido sobretudo a nível radial, pode significar que este tipo de função seja mais influenciada pela carga que a longitudinal. Estes resultados estão de acordo com o trabalho de Caim et al (52), que realça a independência da pós-carga das velocidades sistólicas longitudinais regionais, mas em oposição a trabalhos de Oki (51) e Mishiro (21), que sugerem que as velocidades sistólicas são influenciadas pelo aumento agudo da pós-carga (Fig. 1 and 2). With regard to regional diastolic function, our results show that this differs significantly from the non-obstructive forms, mainly in terms of velocities and integrals of the a wave, which are lower in the HOCM group, with e and e/a also slightly higher in this group, which may reflect the influence of load conditions on these indices. The higher percentage of segments with e/a equal to or greater than 1 in the obstructive group seems to support this reasoning, although the fact that these alterations are only seen in the septum and the anterior wall would suggest that load dependency and regional pseudonormalization are more marked in these walls (Fig. 1 and 2). Another difference in diastolic function between the groups was seen in the meridional gradient of e in the long axis, which was lower in the HOCM group, suggesting equalization of basal and medial velocities induced by obstruction. Neste contexto, a maior percentagem de segmentos com e/a maior ou igual a 1 no grupo obstrutivo parece apoiar este raciocínio, embora o facto de estas alterações só se verificarem a nível do septo e parede anterior pareça sugerir que a dependência da carga e «pseudonormalização regional» seja mais evidente a nível destas paredes (Fig. 1 e 2). Uma outra diferença na função diastólica entre os grupos verificou-se ainda a nível do gradiente meridional de e no eixo longitudinal, menor no grupo MHO, sugerindo equalização de velocidade basais e medianas induzidas pela obstrução. Mais uma vez, os nossos resultados estão de acordo com os descritos na bibliografia consultada (19-20), que conclui que a função regional por DT apresenta alguma dependência das condições de carga, apresentando a análise efectuada a nível do anel mitral maior independência destas condições (19-20). A proporção entre doentes com MHO e MHNO no nosso estudo é diferente da descrita para a população geral com MH, sendo as formas obstrutivas mais frequentes na nossa população. Este facto relaciona-se provavelmente com o grupo etário analisado, mais idoso, já que as alterações morfofuncionais do envelhecimento predispõem para a obstrução, que é assim mais frequente em idades mais avançadas (1-3). Uma limitação deste trabalho é a não inclusão de segmentos apicais e a exclusão, na which concludes that regional function assessed by TDI is to some extent dependent on load conditions, while analysis at the mitral annulus is more independent of these conditions (19-20). The relative numbers of patients with HOCM and NOHCM in our study are different from those reported for the general population with HCM, the obstructive forms being more common in our population. This is probably related to the more advanced age of the population of this study since age-related morphological and functional alterations predispose towards obstruction, which is thus more common in older patients (1-3). A limitation of this work is the fact that it does not include apical segments, and that the lateral wall and inferior septum (parallel to the ultrasonic beam) and the anterior septum are excluded from the analysis of radial function. However, all these segments were also excluded from the multicenter MYDISE study (53), with the exception of the anterior septum, which was also excluded in our study because of the poor technical quality of the recordings. Another limitation of this work is that it does not include patients with severe mitral regurgitation, which could influence myocardial velocities. However, despite this exclusion, the HOCM group still presents indications of increased preload, since it includes a larger number of patients with pseudonormalization and a higher percentage of patients with moderate mitral regurgitation.
