Actualmente, a abordagem percutânea permite ao electrofisiologista reproduzir com eficácia esta estratégia ablativa, vindo a constituir-se como a principal alternativa não farmacológica para o tratamento da fibrilhação auricular. Descreve-se o caso de um homem com 46 anos de idade, com fibrilhação auricular paroxística idiopática, sendo os episódios sintomáticos muito frequentes e refractários à terapêutica farmacológica. Efectuou-se estudo electrofisiológico em Janeiro de 2002. Após ter sido colocado um electrocateter no seio coronário, outro, decapolar (Lasso ) no ostium da veia pulmonar superior direita e um terceiro, de ablação (NaviStar ) na proximidade desse local (Fig. 1), identificou-se um potencial bifásico que iniciou um Isolation of the pulmonary veins with radiofrequency energy for the treatment of atrial fibrillation was initially described by our group as a surgical technique (1). The percutaneous approach now enables the electrophysiologist to successfully reproduce this ablation strategy, which has become the main non-pharmacological alternative for the treatment of atrial fibrillation. We describe the case of a 46-year-old male patient with idiopathic paroxysmal atrial fibrillation, whose symptomatic episodes were very frequent and did not respond to pharmacological therapy. An electrophysiological study was carried out in January 2002. An electrocatheter was first placed in the coronary sinus, then another, decapolar (Lasso ), was positioned in the ostium of the right superior pulmonary vein and a third, ablation catheter (NaviStar ) near the latter site (Fig. 1). episódio de fibrilhação auricular (Figs. 2 e 3). A ablação neste local com energia de radiofrequência, permitiu converter transitoriamente a arritmia (Fig. 4). Decidiu-se, por isso, isolar essa veia pulmonar segundo a técnica designada por desconexão eléctrica (Fig. 5) (2). Na fase final do isolamento da veia pulmonar, verificou-se durante a aplicação de radiofrequência, que o coração retomou ritmo sinusal. No entanto, os electrogramas da veia pulmonar superior direita mantinham-se em fibrilhação (Fig. 6). As imagens do caso presente vêm demonstrar o valor terapêutico do isolamento das veias pulmonares. Está estabelecida a importância da actividade ectópica das veias pulmonares na génese de episódios de fibrilhação auricular (3). Neste caso, a ineficácia da ablação directa do foco de iniciação e a restauração do ritmo sinusal durante a desconexão eléctrica da veia pulmonar, evidenciam a vantagem do isolamento das veias sobre a ablação focal. Além disso, a persistência de fibrilhação dentro da veia pulmonar desconectada demonstra que esta não só estava implicada no início dos episódios arrítmicos (extra-sístoles) mas que ela constituía o próprio mecanismo de manutenção da fibrilhação auricular. Os mecanismos electrofisiológicos responsáveis pela fibrilhação auricular não estão ainda completamente esclarecidos. O isolamento das veias pulmonares, além do bloqueio de focos ectópicos iniciadores da fibrilhação auricular, até agora considerado como o fundamento da técnica, pode ter ainda a vantagem de interferir com os mecanismos da sua manutenção. As veias pulmonares constituem assim um factor indiscutível para a manutenção da fibrilhação auricular, a associar a outros mecanismos eventualmente implicados como os descritos pela teoria dos múltiplos circuitos de reentrada (4). fibrillation (Figs. 2 and 3). Ablation at this site with radiofrequency energy led to a temporary conversion of the arrhythmia (Fig. 4). It was therefore decided to isolate this pulmonary vein using the technique known as electrical disconnection (Fig. 5) (2). In the final stage of isolating the pulmonary vein, the heart returned to sinus rhythm during the radiofrequency application. However, the electrograms of the right superior pulmonary vein continued to show fibrillation (Fig. 6). The images in the present case demonstrate the therapeutic value of isolating the pulmonary veins. The importance of ectopic activity in the pulmonary veins in triggering episodes of atrial fibrillation is well established (3). In this case, the ineffectiveness of direct ablation of the focus of initiation and the restoration of sinus rhythm during electrical disconnection of the pulmonary vein are evidence of the advantages of isolating the veins compared to focal ablation. Moreover, the persistence of fibrillation in the disconnected pulmonary vein shows that it was not only involved in the initiation of episodes of arrhythmia (extrasystoles) but that it was in fact the mechanism maintaining atrial fibrillation. The electrophysiological mechanisms responsible for atrial fibrillation are not yet fully understood. Isolation of the pulmonary veins, besides blocking ectopic foci that initiate atrial fibrillation, up to now considered the rationale of the technique, may also have the advantage of acting upon the mechanisms that maintain it. The pulmonary veins are thus undoubtedly a factor in the maintenance of atrial fibrillation, in association with other mechanisms that may be involved, such as those described in the theory of multiple re-entry circuits.
